Job summary:



The successful candidate will be based in a personalised care team in the GP Practice. The Data Analysist Training Role for Personalised Care Plans plays a critical role in supporting the delivery of tailored projects by managing, maintaining, and analysing patient data accurately and securely. This role works closely with the Personalised Care Team Lead and the wider leadership team to ensure that projects and initiatives are data-driven, enabling the practice to effectively meet the evolving needs of its population.

Boroughbury Personalised care team projects are initiatives designed to tailor our registered population's health and social support around an individual's unique needs, preferences, and circumstances. These projects typically involve multidisciplinary teams across the surgery and four sites. We working collaboratively to develop and deliver care that reflect what matters most to our population. Our projects aim to improve outcomes and patient satisfaction.

This role is intended to become an integral part of the Practice's multidisciplinary team, working alongside social prescribing link workers, care coordinators, Health, Recovery and Wellbeing Lead and support the Health inequalities lead and GP to promote and embed a personalised care approach.

You will be a caring, dedicated, reliable and person-focused individual who enjoys working with a wide range of people. You will have a strong working knowledge of Microsoft Office programs and general IT, good written and verbal communication skills and proven strong organisational and time management skills. As this is a new and evolving role, we are looking for highly motivated and proactive applicants with a flexible attitude, keen to work and learn as part of a team, and who will be committed to providing data driven pathways for our patients.

The post holder will demonstrate excellent organisational and communication skills, be flexible in their approach, able to exercise initiative and demonstrate consistently high standards of professionalism. The post holder must at all times be aware of the need for confidentiality and integrity.

Please note that the role of a Data Analysist Training Role within the Personalised Care team.

- Assist project managers with planning, scheduling, and monitoring project tasks and timelines.

- Maintain project documentation, including status reports, meeting minutes, and action logs.

- Coordinate project meetings, prepare agendas, and distribute meeting materials.

- Track project milestones and deliverables, flagging any delays or risks to the project manager.

- Support preparation of presentations and reports to update leadership and board management on project progress. This will include ICB and Integrated neighbourhood.

- Manage project-related administrative tasks such as filing, data entry, and maintaining project management tools/software.

- Help identify process improvements and contribute to best practices in project execution.

- Provide training or support to staff completing work related to the projects and joy platform.

- Providing population health promotion insights to support strategic planning, resource allocation, and performance improvement initiatives.

- Ability to work within a multidisciplinary team and multiagency team

All staff has a responsibility to undertake statutory and mandatory training as deemed appropriate

In addition to the duties and responsibilities listed above, the post-holder may be required to perform other duties assigned by the supervisor/manager from time to time

Education, Qualifications and Training



Effective communication skills to work with multiagency team, and ability to work independently. A focus on professionalism and confidentiality.

Strong organisational skills and ability to manage multiple data requests efficiently.

4 GCSE's grade A*- D

Awareness clinical systems or the desire and ability to learn new systems, pathways and new roles quickly will also be considered

Interest in development and progressing the role to become a data analyst via a Level 4 Data Analysis Apprenticeship

Relevant IT qualification or willing to work toward

Experience in data extraction, data manipulation, or data within S1 and Eclipse, data collection tool.

Experience with data visualisation tools and techniques

Familiarity with clinical systems and be able to extract relevant patient data from clinical systems and digital systems to support service delivery, those with the desire and ability to learn new systems quickly will also be considered

Understanding of healthcare data management and reporting, there will be training provided if not

Strong attention to detail and ability to maintain data integrity.

Experience



Demonstrate an understanding of customer care when liaising with the public on the telephone, electronic and / or face to face

Ability to communicate effectively and sensitively

Experience of team working

Knowledge of Personalised Care Team Module and patient focused delivered care

Awareness of CQC and QOF standards

Awareness/Experience in supporting audit and compliance activities.

Knowledge and Skills



Ability to work under supervision and confidence to seek advice when appropriate

Accuracy and attention to detail

To be able to work to deadlines

Basic computer skills i.e. able to use email and internet to send and retrieve information

Ability to maintain strict confidentiality at all times

Able to carry out basic clerical tasks following instruction
