Dementia Outreach Coordinator
to join our team and help deliver meaningful support across the community.
About the Role
As Dementia Outreach Coordinator, you'll play a vital role in supporting individuals living with dementia and their carers through group activities, tailored one-to-one sessions, and community engagement. You'll lead initiatives such as:
Dementia activity groups and carer support groups
Maintenance Cognitive Stimulation Therapy (MCST) sessions
One-to-one respite and activity sessions in homes and community settings
Building partnerships and coordinating volunteers to ensure smooth delivery
Essential Requirements
Experience working with people living with dementia and their families
Experience in adult safeguarding and delivering high-quality customer service
Excellent communication and interpersonal skills
Strong organisational skills and ability to manage priorities independently
Proficient in MS Office and Database Management
Ability to work autonomously and manage volunteer and partner rotas
Caring, compassionate, and patient approach
Confident car driver with access to a vehicle for work purposes
Commitment to equality, diversity, and continuous learning
Willingness to undergo an Enhanced DBS check and Right to Work verification
Desirable
Current DBS registered on the update service
Previous experience delivering MCST or similar therapeutic programmes
Working Conditions
Travel across community locations is required
Lone working may be involved, following Age UK Surrey's guidance
