Job Title: Dementia Support Worker
Reporting to: Dementia Services Manager
Hours of Work: Monday to Friday (occasional weekend work as required) 9.30am till 3.30pm
Salary: 12.32 per hour
Main Purpose of the Role
To assist in the delivery of activities and services provided by Age Concern Sandwich for individuals living with dementia, supporting their independence, wellbeing, and community inclusion.
Key Responsibilities
Provide companionship to dementia clients in the Sunflower Centre.
Support Team Leaders in the delivery of a variety of client-focused activities.
Conduct welfare checks to ensure the well-being of service users.
Encourage and support clients to pursue hobbies and maintain interests.
To assist service users with personal care, respecting the person's dignity, independence and right of choice at all times.
To assist with serving of meals and drinks as required.
To ensure that all service users are dealt with in a courteous, friendly and efficient manner that respects their dignity, independence and right of choice.
To comply with the appropriate procedures for reporting and record keeping, including the secure maintenance of service users' records.
To record all monies and expenditure in accordance with set procedures.
Facilitate access to community groups and services.
Assist with escort duties in Age Concern Sandwich vehicles or alternative transport.
Contribute to planning and implementing meaningful, person-centred activities.
Provide flexible support across departments during staff absence or high service demand.
Maintain awareness of how long-term health conditions impact individuals and carers.
Empower clients to take control of their health and wellbeing.
Participate in training, development, and governance activities as required.
Immediately report concerns regarding client health, welfare, or safety in line with policy.
Support clients in maintaining independence and accessing dementia-friendly resources.
Promote awareness and reduce stigma associated with dementia within the wider community.
Facilitate choice, independence, and informed decision-making under the Mental Capacity Act (2005).
Escalate risks to appropriate medical professionals, including GPs or Mental Health Teams.
Support clients in accessing appropriate health, treatment, and benefit entitlements.
Maintain confidentiality in all aspects of work and ensure accurate client records.
Tailor information provided to suit individual dementia pathways and family needs.
Promptly report incidents or accidents affecting client safety.
Demonstrate self-motivation and responsiveness to client and organisational needs.
Operate within Age Concern Sandwich's policies, procedures, and quality standards.
Participate in fundraising efforts and positively promote the organisation's services.
Collaborate respectfully with management and all internal departments.
Ensuring rooms are kept clean and tidy.
Person Specification
Essential:
A strong commitment to empowering older people and supporting their independence.
Knowledge and understanding of issues affecting older adults, dementia, and carers.
Experience in mental health, particularly dementia care.
Understanding of key legislation, including the Mental Capacity Act, Care Act, and Mental Health Act.
Good knowledge of adult safeguarding principles and procedures.
Excellent interpersonal and communication skills.
Ability to prioritise workload and work independently.
Flexibility to meet the changing needs of the service.
Commitment to confidentiality, professionalism, and service values.
Punctual, honest, and dependable.
Ability to contribute effectively to a team environment.
Desirable:
NVQ Level 2 in Health and Social Care or equivalent.
Experience working with statutory, voluntary, and private sector partners.
A person-centred, empathetic, and patient approach.
Strong customer service orientation and community engagement skills.
Basic Food Hygiene certification.
