Job Description

Main Purpose of the Job:




This is a newly created post funded through the Big Lottery Community Fund.


To provide tailored support, information, and guidance to people living with dementia and their carers, helping them to safely participate in the Big Lottery Community Fund Active Connections project. The role will focus on supporting individuals and families to identify their needs, access appropriate elements of the Active Connections project, and engage in meaningful activity.

Dimensions and Limits of Authority



Responsible for delivering high-quality dementia support services in line with Age Connects Torfaen values and standards. Accountable for liaising with statutory, voluntary, and community organisations to ensure holistic and effective support. Maintain professional boundaries while providing practical and emotional support to individuals and carers.

Duties and key responsibilities:



To manage referrals, assisting clients in assessing dementia services. To provide respite provision for carers as and when required. To maintain and manage a caseload of clients who have dementia or are affected by dementia from appropriate geographical locations within Torfaen and the surrounding boroughs. To support people with dementia of all ages and varying needs within all Dementia services including; Merry Moments, Social Circle, Cognitive Stimulation Groups, Music & Movement and Active Connections. To provide daily activities that are engaging and stimulate the needs of a person with dementia. To support 1-1 activity sessions in the clients homes that are engaging, client led and stimulate the needs of a person with dementia. To provide information regarding Age Connects Torfaen services making the appropriate referrals as and when needed. To provide an innovative person centered plan for each client and to keep this regularly update to their changing needs. Maintain accurate, timely records using Charity Log and ensure all client information is handled in line with GDPR and confidentiality requirements. When appropriate, to signpost clients to a choice of suitable other sources of help. Actively reach out to people affected by dementia who may be isolated or not currently accessing support. To develop networks with partner organisations who also work in the field of dementia, mental health, disabilities, and benefits and income advice, including Primary and Secondary care, Social Care and Health, and Welfare Benefit agencies, with the aim of empowering individuals to make informed choices. To assist in raising public awareness about dementia and the way it affects families and individuals, and to participate in local events such as conferences. Work in partnership with health, social care, and voluntary sector partners to signpost, advocate, and empower individuals and families. To recruit, train and support volunteers to become specialist dementia volunteers

Person Specification - Dementia Support Worker



Criteria



Essential



Desirable



Qualifications





NVQ/QCF Level 3 in Health & Social Care or equivalent.

Dementia-specific training (e.g. Dementia Friends, CST training).



Good standard of general education.

Willingness to undertake dementia and safeguarding training.

Experience





Experience of working with older people, people living with dementia, or vulnerable adults.

Experience of providing one-to-one support and group activities.



Previous work within a dementia service or health & social care setting.

Experience of working with carers and families.

Knowledge





Understanding of dementia and its impact on individuals and families.

Awareness of safeguarding and confidentiality principles.



Knowledge of local services, benefits, and support networks.

Skills





Excellent communication and interpersonal skills, with the ability to build trust and rapport.

Strong organisational and time management skills.

Ability to work independently and as part of a team.

Good IT skills, including use of databases (e.g. Charity Log) and Microsoft Office.



Ability to deliver presentations and represent the service at events.
