An exciting opportunity is available with the Diabetes Team at Glangwili General Hospital. Are you an experienced and accomplished diabetes specialist nurse?



The post holder will work collaboratively with patients, patients family, public and professional staff across a spectrum of health and social care services.



The post holder will be expected to lead, develop and deliver a high standard of diabetes services aiming to promote health, enabling and encouraging the individual with diabetes to achieve the best possible self-management of diabetes utilising skills and knowledge in consultation with the multidisciplinary team (MDT).



We invite you to apply for the Diabetes Specialist Nurse. Extensive post registration experience of working within diabetes care is essential.



Diabetes specialist nurse required with expert and advanced knowledge and experience in the management of patients with diabetes Type 1 and 2, gestational diabetes, pregnancy in pre existing Type 1 and Type 2 diabetes, insulin pump therapy and nurse led clinics.



Post graduate qualification required in diabetes management and prescribing qualification desired.



Work to the level of senior specialist nurse as out outlined by the career and competency framework for diabetes specialist nurse (TREND).



Demonstrate high level of clinical nursing skills and expertise embracing the advancements in pharmaceutical, technological and educational treatment and care of young people and adults with diabetes.



Undertake patient assessment, plan and implement care.



Be committed to enabling people with diabetes to self-manage their condition within their capacity and consent. Embrace the directive from NICE guidance to provide structured education for people with diabetes.



Improve the management of patients with diabetes by optimizing investigations and tests interpretation and acting upon and communicating the results to patients.



Monitor indicators of diabetes, anticipating possible decline and proactively managing patient's wellbeing in order to maintain the patient's independence.



Ensure a safe environment for patients, visitors and staff.



To have the knowledge of all Health Board Policies and procedures/guidelines.



Work with individuals with diabetes with complex health needs and provide support to junior staff within your locality.



Team working with other specialities and Health Care professionals in the field of diabetes to provide the best possible service and improve the individual's diabetes journey.



Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.



We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.



We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.



Our services are provided in:



Four main hospitals:

Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest

Five community hospitals:

Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire

Two integrated care centres:

Aberaeron and Cardigan in Ceredigion, and several other community settings

47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers

Numerous mental health and learning disabilities services



The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
