Job Summary:
We are looking for an experienced and forward-thinking Diabetes Nurse Specialist to take on the role of Clinical Lead for Diabetes within our general practice team. The post holder will lead on the development, delivery, and evaluation of a high-quality, evidence-based diabetes service, in line with NICE guidance, QOF, DES/IIF targets, and local ICB strategies.
You will combine expert clinical care with leadership responsibilities to coordinate the diabetes pathway, guide service improvement, and act as a source of expert knowledge and support for colleagues across the practice team.
Key Responsibilities:
Clinical Leadership*
Lead on the development, implementation, and review of the practice's diabetes strategy, protocols, and pathways.
Chair regular diabetes team meetings, coordinating input from GPs, practice nurses, HCAs, pharmacists, and allied health professionals.
Ensure the diabetes service is aligned with national guidance (e.g. NICE), local ICB priorities, and best clinical practice.
Act as the key point of contact for external stakeholders (e.g. ICB diabetes leads, community diabetes teams, hospital liaison).
Clinical Practice*
Manage a caseload of patients with Type 1, Type 2, and gestational diabetes, providing comprehensive assessments and care plans.
Initiate and titrate insulin and other diabetes medications, using independent prescribing or under agreed clinical pathways.
Lead nurse-led diabetes clinics, delivering structured education (e.g. DESMOND or in-house), lifestyle counselling, and long-term follow-up.
Monitor and interpret clinical data, such as HbA1c and blood pressure trends, and make evidence-based treatment decisions.
Quality Improvement and Performance*
Oversee diabetes-related elements of QOF, IIF, DES, and local enhanced services.
Lead clinical audits and service evaluations, using findings to shape improvements in patient care and safety.
Ensure accurate and timely clinical coding and contribute to annual reviews and chronic disease recall systems.
Team Development and Education*
Provide leadership, mentorship, and clinical supervision to nursing colleagues, HCAs, and students with a focus on diabetes care.
Deliver in-house training sessions and support the professional development of the wider team in diabetes-related topics.
Promote a culture of learning, reflection, and evidence-based practice.
Postgraduate qualification in diabetes care (Essential)
Independent Nurse Prescriber (Essential for clinical leadership role)
Leadership and/or mentorship training (Desirable)
Experience:*
Substantial post-registration experience with a strong focus on diabetes care (Essential)
Experience in a primary care or community setting (Essential)
Proven experience in a leadership or service development role (Essential)
Experience managing QOF, IIF, or audit processes (Desirable)
Skills & Attributes:*

Excellent interpersonal, organisational, and communication skills
Proficient in using SystmOne, or similar systems
Committed to continuous professional development and quality improvement
Additional Information:
Based in general practice; may include occasional outreach work, MDT meetings, or joint clinics.
Required to undertake regular CPD, attend local ICB updates, and contribute to revalidation processes.
