We have a rare and exciting opportunity for an enthusiastic, highly specialist, and proactive Registered Dietitian to join our innovative community team. This role is central to delivering advanced dietetic services for people with diabetes across Merton and Wandsworth, South West London.

Integrated Multidisciplinary Team:

Work alongside Consultants, Specialist Nurses, and Dietitians, collaborating on complex patient care pathways.

We manage people with both Type 1 and Type 2 diabetes and are an initiating centre for Freestyle Libre technology.



Structured Education Expertise:

You will be integral to the delivery of established, evidence-based structured education programmes like DAFNE and DESMOND .



Community Based:

The role is community-based, requiring travel between CLCH sites in South West London to ensure care is delivered where it is needed most.



The Community Diabetes Service requires a specialist clinical Dietetic Diabetes Service that contributes to the strategic planning and delivery of integrated care across Tiers 1-3.



As a key member of the multidisciplinary team, you will be responsible for the full specialist scope of dietetic diabetes care:



Specialist Clinical Practice :

Provide expert assessment and treatment for clients with both Type 1 and Type 2 diabetes, ensuring high-quality, evidence-based care.



Service Contribution:

Contribute to the strategic direction of the service, aligning with Trust objectives and national guidelines (e.g., Diabetes NSF, NICE guidance).



Partnership and Development:

Utilize project management skills to plan, develop, implement, and evaluate service developments and interventions in partnership with service users and other healthcare providers.



Interagency Liaison :

Effectively liaise with primary care, community services, and local acute hospitals to ensure seamless, integrated patient pathways.



Education and Training:

Deliver specialist nutrition training and education for diabetes to service users, staff, and other stakeholders.



Community Engagement :

Engage with and support local initiatives, such as support groups and Diabetes Champions, according to NICE guidelines.



Record Keeping:

Maintain accurate and timely clinical records.
