We are seeking a dedicated and knowledgeable Diabetes Specialist to join our healthcare team. The successful candidate will play a vital role in providing specialised care and education to patients managing diabetes. The Role
You will play a key role in delivering evidence-based diabetes care in line with QOF, NICE guidance, and local pathways. Working independently and collaboratively with GPs, pharmacists, HCAs, and the wider MDT, you will support our diabetic population through structured reviews, patient education, and personalised care planning.
Responsibilities

Deliver comprehensive diabetes clinics, including annual reviews and interim follow-ups
Initiate and manage insulin and other diabetes medications within competency
Provide individualised patient education and self-management support
Interpret and act on diagnostic tests (e.g., HbA1c, lipids, kidney function)
Work closely with the practice team to optimise QOF outcomes and reduce health inequalities
Contribute to recall systems, long-term condition pathways, and proactive population management
Maintain accurate and high-quality clinical documentation using EMIS
Participate in MDT meetings and quality-improvement initiatives
Qualifications
Registered Nurse with current NMC registration
Experience in diabetes management in primary care or community settings
Non-medical prescribing qualification (or willingness to work toward it) desirable
Confident in insulin initiation/titration, or working toward competence
Strong communication, organisational, and clinical decision-making skills
Passionate about improving outcomes for patients with long-term conditions
