Job Description

Main duties and responsibilities To demonstrate a professional approach to the leadership and management of the in-patient service. Assumes accountability and responsibility for practice and along with the members of the healthcare team, strives to attain the highest standard of practices. Nurse/Patient relationships Demonstrates and establishes a relationship of trust with patients, carers, and families. Demonstrates expertise when dealing with complex patients with co morbid conditions and their families who require new skills to live with a chronic condition.

Management of patient ill health/illness status Receives direct referrals from medical staff, nursing staff and patient self-referral. Is able to review appropriateness of these referrals. To act as an expert practitioner in the management of the patients chronic condition, demonstrating advanced clinical skills and knowledge to professional staff and patients with a diabetes condition. Education Identifies the ongoing training and educational needs of the person with diabetes, as well as their carers and their families in relation to the treatment and management of different types of Diabetes and the prevention of related complications.
