Job type: one year fixed-term contract

Pay: Up to 26,676.00 per year

ABOUT THE SERVICE & ROLE



The Digital Health Service provides a mechanism for delivering 21st Century care and connecting people with medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.

The Clinical Support Team Member will assist in clinical case management, using virtual technologies, for patients living with short- and long-term conditions. The Digital Health Clinical Support Team Member is a pivotal support role for assisting with monitoring of patients on the caseload and responding to markers of declining patient health. Escalation of concerns to The Digital Health Clinicians ensures intervention is delivered in the community setting; consequently, helping the patient regain stability, and preventing in-patient hospital admission.

KEY RESPONSIBILITIES



The post holder will:

1. Have an appropriate experiential health care background such as: health care assistant, therapy assistant, emergency medical technician.

2. Work with The Digital Health Clinicians supporting high level remote monitoring for patients with short- and long-term conditions. Escalate to The Digital Health Clinicians when markers for deterioration are evident on submission of abnormal observations by patients via the Luscii system. Thereby, ensuring patients on the caseload who alert have clinical contact urgently. This ensures care is delivered in the community setting and aids prevention of unnecessary hospital admission.

3. Work with partners in nursing and residential care to ensure optimum health outcomes for their residents.

4. Work on shared caseloads with Integrated Care System (ICS) by acting as an integral member of the community team with a shared vision for developing caseloads as required by Integrated Care System partners.

5. Possess experience working within the telemedicine field to support the delivery of clinical care remotely; working, under direction of the Digital Health Team Leader and the Digital Health Clinical colleagues when required.

6. Deliver health promotion advice under supervision from clinical colleagues via the Digital Health model, e.g. chronic disease management self-help signposting.

7. Work in alignment with Primary Care, for established practice populations, toward a shared vision of seamless management for patients with short- and long-term conditions within the Primary Care and the Community setting.

8. Work with ICS partners in Primary and Social Care to further develop the Digital Health model for cohorts of patients managed within local populations.

9. Maintain accountability for practice to the Digital Health Team Leader following all EBPC Policies and Procedures.

10. Maintain legible, accurate and up to date records that are dated and signed in accordance with EBPC Policies for standards on record keeping.

11. Contribute to the onboarding of patients with short- and long-term conditions to the patient management platform.

12. Contribute to the discharging of patients with short- and long-term conditions from the patient management platform.

13. Work in a supervised capacity with patients, their families and carers managing referrals according to need and priority.

14. Escalate complex clinical data that has alerted on the patient management platform to the Digital Health Clinical colleagues.

15. Understand and support the achievement of EBPCs business plan objectives and performance targets.

16. Identify the potential for service developments, risks and deficits and inform the Digital Health Team Leader making recommendations based on vocational knowledge.

17. Be accountable for practice taking every reasonable opportunity to sustain and improve knowledge and competence.

18. Assist the Digital Health Team Leader to manage the effective use of resources within the Digital Health team and participate in regular review meetings with the Team Leader.

19. Have experience of Luscii patient management platform or similar.

20. Have experience with other clinical patient management systems: Adastra, EMIS, Connected Care

PATIENTS CARE

The post holder will:

1. Use telemedicine knowledge and skills to remotely monitor the physical and psychosocial needs of a defined client group.

2. Use telemedicine and healthcare knowledge to respond to alerts raised by the patient management platform. Urgently, escalating patient alerts to the Digital Health Clinicians to action.

3. Work with partners in nursing and residential care to improve the health outcomes of the residents and so prevent unnecessary hospital admissions or extended in-patient care episodes.

4. Assist the Digital Health Clinicians who are dealing with complex patient situations. Thereby aiding the establishment of a therapeutic relationship with individual patients using The Digital Health service.

5. Use telemedicine assessment tools/skills that will ensure an appropriate level of therapeutic supervision so that patients who present with complex needs are referred/escalated via the appropriate care pathway.

6. Conduct patient encounters with sensitivity and to act as patient advocate, maintaining patient confidentiality.

7. Deliver care that is patient centred, respecting diverse cultural backgrounds, and requirements.

8. Work in partnership with the patients to empower them to make informed choices about their healthcare and support choices.

9. Offer a supportive service to patients and their carers who are using the Digital Health service

RESPONSIBILITIES FOR PEOPLE AND TRAINING

1. Be responsible for providing and maintaining a learning environment and maximise opportunities for education and development in the clinical area to enhance individual development and performance in the delivery of high standards of care.

2. Act as a Digital Health resource and assist in mentoring new Clinical Support Team Members in the Digital Health environment. Take part in the process of clinical supervision and participate in clinical supervision and mentorship of others.

3. Take responsibility for your own personal development; maintain competence, knowledge, and skills commensurate with role.

JOB REQUIREMENT



Education, Qualifications and Training

Vocational qualification or experiential knowledge for remote clinical management of patient cohorts using Luscii technologies or similar Experiential knowledge of delivering services via telehealth technologies to patient cohorts using Luscii technologies or similar Healthcare support worker background (EMT, HCA etc)
Experience

Significant experience of working with a multi-disciplinary team using virtual solutions/packages to provide care Experience of monitoring patient cohorts using virtual solutions/packages Experience within a wide range of patient/client care groups Community services experience
Knowledge and Skills

To be able to communicate with people from a wide range of backgrounds To be able to work flexibly and sensitively Wide range of interpersonal and communication skills Competence in a wide range of healthcare work I.T. literate Ability to sensitively work with service users To be flexible and adaptable
