We are looking for a compassionate and highly motivated, Support Practitioner to join our dynamic organisation to work with adults with additional Support needs 18yr+
You will be part of a team providing safe, person-centred support to individuals, helping them achieve their personal goals, access their communities, and participate in group-based activities while ensuring dignity, respect, and wellbeing at all times. We are looking for a full time member of staff only, 9:30-3:30pm Monday -Friday (30hrs per week)
ABOUT POSITIVE PATHS
Positive Paths is a Day and short break service, supporting adults with additional needs and differing abilities. We provide community-based programmes that promote personal development, independence, and social inclusion. Our service encourages ongoing learning, confidence building, and opportunities to explore volunteering and supporting members to stay active, connected, and engaged in their communities.
Key Responsibilities
Provide person- centred support in a small group or 1:1 setting, within the local community or in our Base.
Working closely within a team to meet the needs and goals of our adults with additional support needs that attend our service daily.
Supporting the members to build positive relationships with other members and staff, developing friendship groups. (activities like pamper group, karaoke, work experience, yoga and meditation)
Following the Health and social care standards promoting members dignity, choice and independence.
Keeping clear and up to date records of the members support and daily observations.
Planning with the members regarding activities and meeting the members needs and goals, looking at promoting health life choices (cooking healthy meals, being active with swimming, cycling, kickboxing, Dancing)
Keeping up to date with continuous training and development.
Skills, Qualities and Qualifications required
Must have a SVQ 3 Health and Social Care or equivalent qualification or higher
Strong team player with a proactive, positive attitude
Ability to use initiative and problem solving skills
2 + years experience within the Additional Support Needs (ASN) /Disability field or similar (or studies connected to the field)
Motivated and professional approach to supporting individuals with Additional Needs
Excellent communication, interpersonal and IT Skills
Ability to work independently and as part of a team
Commitment to equality, dignity, and inclusion
Confident and able to lone work on 1:1 basis at times
Motivated to provide safe, enriching experiences
Good problem-solving skills and awareness of risk
DESIRED
Full Clean Driving License
Experience handling wheelchairs
Experience of facilitating groups
Working knowledge of Google Drive
Availability for additional midweek or weekend overnights
Epilepsy training
Development & Progression
We actively support staff development and offer clear progression routes, including:
Promotion to Senior Practitioner (based on qualifications, performance, and values)
Opportunities to become a Champion in areas such as: Mental Health First Aid
Training during 6 months induction period
Further ongoing training throughout the year
Supervision & Support
4 face-to-face supervisions per year
Monthly team meetings and supervision
Annual appraisal following successful probation
On-call management support available at all times

Pay: From 13.45 per hour
Expected hours: 30 - 35 per week
