The Role




As a

Discharge Coordinator Nurse

, youll support the Clinical Lead in the day-to-day running of the service, ensuring safe and effective patient care across community and hospital settings. Youll deputise for the Clinical Lead when needed, help manage patient flow with the wider NHS team, and may be required to take part in the

on-call rota

when necessary to support service continuity.

Key responsibilities include:



Work closely with hospital staff to facilitate the effective flow and discharge of patients from hospital into our community teams. Responsible for conducting a holistic clinical patient assessment and coordinating discharge plans, including identifying and resolving internal discharge delays. Identifying care needs and providing support and advice for all aspects of patient discharge. Conducting comprehensive clinical assessments of potential patients, reviewing care requirements to determine suitability for transition to the community setting, including virtual ward and early supported discharge pathways. Performing face-to-face reviews of patient medical history, liaising closely with the Trusts MDT (Multidisciplinary Team), and documenting clinical handovers in real time. Providing support, advice, information, and education to ward staff and multidisciplinary teams to facilitate a safe and timely discharge back to the community. Undertaking all aspects of clinical duties as an autonomous practitioner. Educating patients and families, enabling them to make informed choices in the planning of discharge.

What Youll Need



Active NMC registration At least 3 years post-registration experience as a registered nurse Previous experience in management or supervising staff is desirable Full UK driving licence and access to a vehicle Right to work in the UK (sponsorship not available)

About You




Were looking for compassionate, collaborative, and patient-focused professionals who can lead by example. Youll have excellent communication skills, the confidence to work autonomously across both hospital and community settings, and a commitment to delivering high-quality care.

Recent experience with a broad range of conditions in adults Knowledge of the legal framework under the Care Act 2014 and duties relating to hospital discharge and other legislation relevant to discharge planning Right to work in the UK without any form of sponsorship Have a drivers licence and access to their own vehicle
