Where you will be working



You will have the opportunity to work in one of our Primary Care Network (PCN) Teams covering Bexhill, Hastings / St Leonards and Rural Rother. and Eastbourne, Hailsham & Seaford



You would need to have access to a car and drivers licence for this community role.

What you will be doing



A District Nursing qualification (DN SPQ) is an essential criteria.



Assessing new patients for nursing care in their own homes, creating and evaluating care plans.



Managing a caseload with support of the Clinical Lead



Line Management and leadership of a team of Registered Nurses & HCAs.



Our core service hours are 08:00 to 18:00, 7 days per week.



It can be quite a challenging role at times, so a positive attitude, compassion, and resilience and vigilance are important.
