To participate in the delivery of a flexible Home Care Service which rehabilitates, supports, and enables the continuation of care for vulnerable people living in the community. To assist service users in reaching their potential by helping them to maintain life skills and to enjoy a valued life within their community. To promote the values of respect and dignity for each individual service user's rights and needs. To ensure that all services are provided in an anti-discriminatory manner, by acknowledging and regarding every individual service user's cultural background, age, gender, disability, sexuality, religious beliefs, and social class.
REQUIREMENTS FOR HOME CARE
That you can demonstrate 'Good Practice' with regards to the care and support of service users in the community and in a residential setting.
That you are reliable, honest, have good time-keeping skills and can follow written and verbal instructions.
That you undertake the service requirements as detailed below under Main duties and Responsibilities.
MAIN DUTIES AND RESPONSIBILITIES
To attend to the personal needs of the service user as agreed with the manager, and as detailed in the Service user's care plan and to carry out all work in accordance with the Yeshua Healthcare care worker induction Manual and Company policies and Procedures
To ensure that the service provided is of a high standard and maintains service users and carers routines and allocated times, therefore not disrupting their normal pattern of living.
To observe, monitor, and feedback on any changes in the service user's circumstances, as soon as practicable, to the manager.
To attend service users, and review as required. When relieving other care workers, ensure that services delivered are in accordance with the service user's agreed care plan.
To ensure that service users receive the necessary support, both socially and emotionally, in a way that is culturally and ethnically sensitive, and reflects the service users' needs in relation to their age, gender, sexuality, social class, disability, or religious belief.
To maintain service user records and information in accordance with Yeshua Healthcare's confidentiality policy
To ensure that you meet CQC standards when delivering care.
To enable the service user to integrate into society and live more independently
To attend to service users' needs and assist them in everyday living
To deal with the social worker, family members and other agencies to ensure that the service user is receiving the best support and care
To prompt the service user to take food and medication if stated in the care plan. Must adhere to the care plan and CQC regulations.
To ensure that the service user is safe and in good health, and to report to the relevant team if the service user is in danger.
To make sure that Food & Hygiene and Health & Safety standards are met. Seek knowledge of any changes in legislation and keep up to date with CQC requirements.
To participate in service user's activities
To assist in the office by carrying out client initial assessments, risk assessments, care planning, evaluation, and audits.
