Duties

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor clients and observe for any changes in health or behavior, reporting findings accurately Administer medications following prescribed care plans if required Assist and prepare nutritious meals and assist with meal planning to meet individual dietary requirements Support clients with assisted living activities such as light housekeeping, cleaning, and maintaining a safe environment Use specialized equipment to ensure safe transfers and mobility assistance Document care activities, update care plans regularly, and communicate effectively with healthcare teams and family members

Experience

Prior experience working in home & community care or assisted living environments is highly desirable Knowledge of dementia, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism Hands-on experience with patient observation, monitoring vital signs, and administering medications Certification in CPR and First Aid is preferred; ongoing training in patient care techniques is advantageous Experience working within nursing homes or long-term care settings demonstrates a strong foundation in senior and resident-centered care Ability to work independently while following detailed care plans and adhering to HIPAA standards for patient confidentiality
