Home Recovery; Hospital Discharge Care Worker



Hours: Full time, AM and Lunch which starts 6AM till 2PM /Tea and PM which starts from 2.30PM till 10.30PM



Areas: Covering South Central Bedfordshire areas including Milton Keynes, Flitwick and surrounding areas.



Essential criteria: Must be a driver with a UK driving licence and use of own vehicle. Must have previous experience working in community care (domiciliary care).



Applicants that have not worked in community care before will not be considered for this role.



A little about the role...



Our team improve and maintain high standards of care, providing the person with an overall positive experience. To provide a responsive service to individuals and families for people discharged home from hospital.

Hospital discharge carers will always ensure high standards, lead by example, provide exemplary care, acting as a role model. They will carry out home visit for short term hospital discharge care packages, supporting the person to improve over a 4 week period.

The hospital discharge team receive and start referrals throughout the week and enable people once back home to find their strength and routine again. The regular referrals help prevent long stay in hospital for people and supports continuous working hours for staff.

The applicant should be engaging with strong communication skills, experience in care and be proactive helping people recovering from their injury or time spent in hospital.

Successful candidates will have service-specific training, refreshers when requested and be part of a hospital specific care team working with the hospital discharge lead.

The job role includes...



Communications with he office, family and co-workers Administering medication, monitoring and reporting on medication stock or amendments Personal care including full body washing, dressing, toileting and continence support at each visit. Communication with other professionals such as community nurses, occupation therapists and discharge team members Working with the person to achieve their goals such as mobility development, food preparations and daily domestic living tasks Maintain detailed, personalised and accurate records for every visit completed Assess and report risks identified from first home visits you complete and ensuring initial records are completed such as body mapping and environmental risk assessing. Work responsively with the discharge team, supporting fast paced referrals to start the next day and feeding back any concerns to support quick resolutions. Help new carers when they join APT, giving them time, advice, support, and guidance. Responsible for reporting any safeguarding concerns (adults and children) Engaging and encoring a person to achieve progress day by day, week by week before they move on to their main provider.

Full Induction Training will be on last week of September 2025



Competitive hourly pay, annual leave and 4 weekly pay



Shifts availability is AM & lunch (6am-2pm) or Tea & PM (3pm-10pm)



Alternating weekends or 1 day every weekend



Mileage and travel time included within payroll



Job Types: Full-time, Permanent

Pay: Up to 12.81 per hour

Expected hours: No less than 40 per week
