An exciting opportunity for an experienced Nurse has arisen to join our Primary Care Network team. They will be working collaboratively with our two Woodley Practices who have a combined population of 30,000 patients.
We are looking for an open and friendly individual who is passionate about delivering high quality nursing care in a dressing clinic setting.
Main duties of the job
The post holder will be responsible for initial assessment, planning, implementation and evaluation of treatment / care plans for a wide range of dressings, including complex wounds.
They will have the ability to work independently under their own initiative and ensure safeguarding needs are met and escalated at the right time.
They will have the flexibility and expertise to provide treatment room services if necessary.

REPORTING TO: CLINICAL LEADS OF EACH PRACTICE
The post holder will be responsible for initial assessment, planning, implementation and evaluation
of treatment / care plans for a wide range of dressings, including complex wounds.
They will work autonomously to improve patient safety & experience, promoting good practice and
promoting skin integrity.
The post holder will have extensive knowledge of wound assessment, be able to identify signs of
infection and use evidence-based knowledge to promote wound healing.
The post holder will actively lead the prevention of avoidable deterioration of wounds and use expert
knowledge and skills to manage complex wound care aiding the recovery and healing.
The post holder will proactively refer patients to the tissue viability service or discharge as appropriate.
They will establish and maintain professional network relationships with all relevant professionals including the Primary Care nursing team and those from local Secondary Care providers.
They will be accountable for their own professional practice and development in line with NMC Code of Professional Conduct and actively participate in clinical supervision.
The postholder will be courteous and respectful to patients and colleagues including adhering to a dress code appropriate to the work situation.
They will maintain adequate dressing services stock.
MAIN DUTIES AND RESPONSIBILITIES
Patient Care
Be clinically inquisitive.
Maintain outstanding clinical documentation, including safeguarding referrals.
Communicate with patients in ways that empower them to make informed
choices about their health and well-being and promote independence where
clinically appropriate.
Ensure the smooth day-to-day running of the dressing clinic using the
dressing clinic standard operating procedure.
Performance and Quality
Attend all Mandatory training and ensure that updates are booked in a timely
manner.
Collect, collate and report on service activities including outcomes data
and audits.
