Location: Aberaeron
Hours: 21 hours per week.
Salary: Starting at 26,265 pro rata
Duration: Fixed term until 31/3/26

Key details/responsibilities:
Ceredigion Trilogy of risk model is one of partnership working with DDAS and VWDAS,(specialist services) and Social Care in order to achieve the desired outcome of early help and prevention of escalation where there is concern surrounding trilogy of risk . The purpose will be to provide specialist Information, advice, and assistance to the person at the point of contact to reduce escalation of needs as support provided at the right time by the right professional. Through providing Information advice and assistance a proportionate assessment will be completed in collaboration with the family using the model of signs of safety. From this proportionate assessment the Triage will identify next steps.
