Carlisle & Brampton Early Supported Stroke Discharge (ESSD) Team Rehabilitation Assistant 20 hours
The ESSD service is offering a rare & exciting opportunity for an enthusiastic, hard working, reliable & flexible person to join the team as a Band 3 Rehab assistant based in Carlisle. The ESSD service provides stroke-specific rehabilitation for patients in the community, facilitating a smooth transfer from hospital to home. The service supports patients to achieve their chosen goals and helps them to integrate back into their communities with the appropriate levels of support for the stroke survivor and their families & carers. The service also helps to prevent inappropriate hospital admissions and promotes a safe & supported discharge from hospital. The service provides high quality, multi-disciplinary (MDT) assessment and treatment, advice/support and / or referral onto other agencies.
The post holder will be part of a 7 day rota system and will be expected to work flexibly across the whole of the stroke patient pathway as well as supporting other teams and services in the delivery of the highest quality patient rehabilitation and care within the West Cumbria Locality.
To work with stroke patients and provide them with the highest quality care, with a rehabilitation and therapy focus, in order to support them to maximise their recovery potential and enable the best possible quality of health and wellbeing after stroke.
To work with stroke patients across the whole of the patient pathway including:
their own home
other community setting such as out-patient clinics, community centres
on any of the acute or community hospital wards
To be able to work unaccompanied as a lone worker, without direct supervision
at times but under the professional guidance and supervision of qualified staff.
To contribute to the assessment, implementation, evaluation and
modification of team interventions.
Use skills to carry out simple assessments and prescribe routine equipment
within the scope of the job and to a level of assessed competence.
Contribute to service development.
To maintain accurate written and computer held records.
Be responsible for daily administration tasks and duties.
To work flexibly to meet the needs of the service and the patients which will
include 7 day working and may include periods of rotation between
community and ward settings.
Support patients with daily tasks using a rehabilitation approach including
personal care, washing dressing bathing etc
The essential criteria for this role includes the requirement to have a care certificate or the willingness and ability to undertake this.
