To carry out delegated duties independently, following an assessment by the Health Visitor (HV) caseload holder, in client's homes and community settings with objectives agreed with the caseload holder.
To work with families to create quality play and learning opportunities for pre-school children.
To screen the growth and development of pre-school children in line with the Healthy Child Programme (HCP) using evidence-based tools.
To deliver evidence-based group programmes for parents and their children with professional supervision and support, in order to ensure fidelity to the defined programmes of intervention.
T To work using the Stockport Family Model of restorative practice, by ensuring assessments and interventions are carried out with the family in a collaborative way.
To contribute to and support supervised contact sessions with the allocated contact supervisor, when the need arises for a family with whom they are working.
To undertake home visits alone or with a colleague.
To carry out any other work deemed appropriate to the role in consultation with line manager.
Main duties of the job
The post holder will work as an integral member of the Stockport Family Service to promote the health, learning and development of children through a variety of interventions and strategies delivered in the home and community settings such as Start Well Hubs, Satellite Hubs , Health Centres and Nurseries. The post holder will work with partners to ensure that services are coordinated and that groups/activities provided will meet the needs of children under five and their families
