Education Health & Care Plan Coordinator

Full time/Permanent/Hybrid

PO2: 43,542 pa incl LW, rising in annual increments to 46,437

Lambeth Local Authority is looking to recruit a Education, Health & Care Plan Co-ordinator to join a friendly and supportive team to deliver an exceptional service in the execution of the Local Authority's SEN statutory responsibilities as set out in the Children and Families Act 2014. Lambeth's cooperative structure aims to strengthen the involvement of local people in the services they receive in partnership with all local providers. Therefore, the Borough has a key responsibility to ensure that there is good quality support for all of its learners, parents /carers.


The EHCP Coordinator is responsible for managing and coordinating Education, Health, and Care Plans (EHCPs) for children and young people with special educational needs and disabilities (SEND). This role involves working closely with families, schools, local authorities, and multi-agency professionals to ensure that children and young people receive the appropriate support for their educational and developmental needs.


The post holder will:
Writing EHC Plans. Coordinate assessments and casework across the SEND Service, ensuring that casework is legally compliant whilst remaining child centred, pro-active and responsive to the needs of children and young people and paying due regard to the views of parents. Coordinate and actively support a person centred, outcome focused multi agency approach to assessment, planning and support, ensuring that children, young people and parents are treated as equal partners in the assessment and planning process. Guiding settings and parents to the right provision at SEN Support level. Directing them to the Local Offer to find more information on the services available. Supporting settings and parents to provide the right information if an EHC needs assessment request is made. Coordinating the 20-week process, liaising with settings, parents and other professionals. Attending EHC plan review meetings, at critical points such as change in phase of education. Supporting settings and families with phase transfer. You will chair and attend annual reviews where there is a high risk of placement breakdown or a complex case within the targeted age groups. You can call upon support from Team Leaders/ Team Manager where required. You will provide information and support around the Local Offer and will be expected to stay up to date with the Local offer itself.

The successful candidate will work with key partners including parent carers, children and young people, colleagues in Lambeth Council, SENCOs of local schools including academies and free schools.
