Job Overview

An exciting opportunity has arisen to join this Hampshire based charity, as part of their Emergency Planning Project Team (supporting the carer rather than the cared for). Covering Hayling Island, Waterlooville and the surrounding areas. The main duties will include:

To respond to referrals within agreed timescales and work within specified timescales when drawing up Emergency Plans.

To support carers, to develop and implement a comprehensive and individually tailored Emergency Plan.

Liaising with colleagues and other organisations that may be able to provide additional support to carers health, social services and other voluntary organisations.

Ongoing updating of the database with client details and Carer Support Workers interventions so that the service can be monitored through statistics, information and reports.

Diary management.

Promoting the service offered via presentations and participating at information days.

Raise awareness of Emergency Planning for Carers with other agencies both statutory and voluntary via various means including giving presentations and work with these agencies toward identifying carers and hard to reach groups.

'Signposting' to other organisations.

Participating in appropriate training and keeping up to date with legislation, local policies and services

Work collaboratively as a member of a team, attend and actively contribute to staff meetings.

The successful candidate will have: -

Experience in health, Voluntary sector or Adult services.

Excellent communication skills.

The ability to manage a varied caseload.

Pay: 25,235.00 per year
