Job Description
We are seeking a dedicated and compassionate Qualified Social Worker (QSW) to join our Adult Social Care team in Enfield. This is an exciting opportunity for an individual who is passionate about improving the lives of adults in our community. As a QSW, you will play a crucial role in assessing needs, developing care plans, and ensuring the delivery of high-quality services that promote independence and well-being.
Your key responsibilities will include:
- Conducting comprehensive assessments of individuals' needs and circumstances.
- Developing and implementing person-centered care plans in collaboration with service users and their families.
- Monitoring and reviewing care plans to ensure they remain effective and responsive to changing needs.
- Providing support and guidance to individuals, families, and carers to navigate social care services.
- Working collaboratively with multidisciplinary teams and external agencies to deliver integrated care solutions.
- A recognized Social Work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registration with Social Work England.
- Experience working in Adult Social Care, with a strong understanding of safeguarding and legislation.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
- Strong analytical and problem-solving abilities, with a commitment to continuous professional development.
