The Pall Mall Surgery are seeking an experienced and motivated enhanced practice nurse with a specialist interest in diabetes to join our team on a part-time basis, for 20 hours per week. The postholder will play a pivotal role in delivering high-quality, evidence-based diabetes care to our patient population, alongside other long term conditions and other practice nurse duties.
The main responsibilities of the role include:
Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
Providing and evaluating a range of health interventions to ensure that patients' health and social care needs are met.
Leading diabetes clinics, providing assessment, diagnosis, treatment, and ongoing management for patients with Type 1 and Type 2 diabetes.
General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
Managing and treating long-term conditions, in line with national and local policies and the practice needs.
Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
Meeting the needs of patients presenting for opportunistic wound care.
Hypertension review and management.
Running asthma, CHD, COPD and diabetes clinics and reviews.

About the Candidate
All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree.
Current NMC registration with an active PIN
Independent non-medical Prescriber
Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
Evidence of clinical skills and experience in treatment room consultation, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, dressing and wound care.
Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
Knowledge, and experience of its application, in supporting long-term conditions.
Insight and understanding of current issues in nursing and the wider NHS.

The Pall Mall Surgery is a large, modern GMS training practice operating from a contemporary health centre in Leigh On Sea which in 2022 was voted the best place to live in Essex. The town has excellent transport links, with two separate train lines into London. The practice is popular in its locality and supports 21,500 patients from birth to end-of-life care. We operate a total triage system at the surgery, which enables us to make quick decisions about patient care and outcomes, ensuring the most appropriate appointment is given to each patient based on symptoms. We offer face-to-face (15-minute) appointments and telephone consultations (10 minutes).
