Salary: 13,458.



Hours: 17.5 pw (evening & weekend work may be required)



Location: Stockport, Greater Manchester



Responsible to: Senior Neighbourhood Coordinator



Additional: Must be a driver and have use of a car for business purposes



Mileage: 45p per mile

Role Purpose:



To work within the Community Hub team & understand the cohort's needs and barriers to accessing services and to act as a point of contact for connecting to relevant services and support.

The Community Hub is part of the Early Intervention & Prevention services in Stockport. The Stockport Support Hub.

Pure's EX Armed Forces Link worker will offer 1:1 support to individuals aged 18 and over in the Stockport area, who have served in the military.

This role involves assisting clients with issues related to social isolation, health and wellbeing. Introduction support to leisure activities, volunteering opportunities, training & learning and community involvement, while helping them address any underlying challenges they may face.

To develop and maintain links with the AF&V community in Stockport.

Our goal is to help people connect with their communities and engage in opportunities that lead to a fulfilling and meaningful life.

Additionally, the post holder will be involved in the facilitation of the Community Hub's weekly social groups, projects and collaboration with other organisations & members of the Network and the community to achieve the service's objectives.

Responsibilities:



Utilise the existing specialisms and workstreams within Pure Innovations to benefit the AF&V community



Carry a small caseload (AF&V) of clients from Com Hub, and MASsH waiting lists.

Monitor and Maintain the AF&V Single Point of Contact for direct referrals and communications

Be the point of contact for Pure's Wellbeing Teams for information and advice to clients regarding AF&V services and activities.

To develop and maintain links with the AF&V community in Stockport to understand needs and barriers to accessing services and to act as a point of contact for connecting to relevant services and support.



Attend existing activity groups and sessions to engage with users to inform future offer/provision.

Represent Pure Innovations at Stockport Armed Forces Community Organisation (SAFCO) members meetings and events.

Identify natural support networks either in the community or in people's own networks of support.

To engage with existing AF&V groups and those organisations offering support to identify potential for joint working



Support start up of new community based social groups and activities in partnership with other organisations .

Support delivery of events and activities to engage the community.

Attend SAFCO general meetings as Pure's representative.

Take responsibility for monitoring and recording data to contribute to organisational reporting.



To keep and maintain records of activity, both written and electronic, and produce regular monitoring information as required.

Compile and report data to line manager & Wellbeing Teams Manager.

Prepare case studies

Actively promote and share activity info and good news stories through dedicated marketing team.

To work as an active member of the Stockport Third sector attend meetings and contribute to the ongoing development of the service.

To undertake and contribute to appropriate training. To participate in a formal programme of 1:1 meetings and annual appraisals. To participate in conferences, courses and training events as required. To maintain an up-to-date knowledge of relevant legislation and service developments and to adhere to all appropriate policies, procedures and guidelines. Person Specification



Experience



Essential



Have lived experience of military life, such as an individual who has served in the armed forces or a relative of someone who has.

Desirable



Been actively involved with the AF community in Stockport

Experience of working with multidisciplinary teams.

Knowledge of Stockport's community locality offers.

Knowledge



Have a good understanding of the AF community in Stockport

Knowledge of conditions and their effect of people's abilities to face daily challenges and overcome social isolation.

Ability to identify risks

Ability to work as a part of a multidisciplinary team.

An understanding of the place of ordinary life principles.

Skills



Evidence of an ability to make sound judgements and decisions on a day-to-day basis.

A range of I.T. skills.

Imaginative/creative approaches to the integration of people with barriers into the local community.

Comfortable with remote/lone working & setting professional boundaries.
