Job Summary:

Vivid Community Care is seeking an experienced and proactive

Care Coordinator

to support our 24-hour care services, including live-in and domiciliary care. You'll be responsible for scheduling care visits, supporting care staff, and ensuring consistent, high-quality care delivery across our client base.

We are based in the

preferably looking for a driver with access to their own car

Key Responsibilities:

Coordinate care worker rotas and daily schedules Ensure care is delivered on time and in line with care plans Liaise with clients, families, care workers, and healthcare professionals Supervise and support care staff Maintain accurate documentation and compliance with CQC standards

Requirements:

Previous experience in a care coordination role (preferably domiciliary or live-in care) Strong knowledge of care planning and scheduling software NVQ Level 3 in Health and Social Care (or working towards) Excellent communication and organisational skills Ability to work under pressure and multitask in a fast-paced environment Must pass an Enhanced DBS check Must provide three years of references Must complete a competency-based interview Driver with own vehicle preferred
