Lincoln House Surgery is looking to welcome an experienced (minimum 1 year), qualified General Practice Nurse to assess, plan, develop, implement and evaluate treatment programmes for our patients.
This role is offered on a Full time or Part time basis for the right candidate, we are looking for a minimum of 4 days per week, 7.5hrs per day, which can be discussed at interview.
Salary dependant on experience (approximately band 6). We are a dual branch practice and you would be expected to work at either branch when required by the needs of the business.
The main responsibilities of the role include:
Undertaking health assessments, administering medication and treatments and performing diagnostic tests;
Providing and evaluating a range of health interventions to ensure that patients' health and social care needs are met;
Health and well-being promotion plus individual person centred plans for patients with/at risk of a long term condition;
Managing and treating long-term conditions in line with national and local policies and practice needs;
Reviewing medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments.
Supporting and managing health needs of women presenting for family planning and cervical cytology consultations;
Implementing and participating in vaccination and immunisation programmes for both adults and children including patients travelling abroad;
Meeting the needs of patients presenting for opportunistic wound care;
Hypertension review and management;
Running Asthma, COPD, Diabetes clinics and reviews;
New patient and health checks.

About the candidate:
If you are a Practice Nurse with GP practice nurse experience, looking for an exciting new role then we would love to hear from you.
You will have the following skills and qualifications:
A relevant nursing degree and at least 1 years post qualification experience in a General Practice setting;
Current NMC registration with an active PIN;
Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications;
Competence to undertake Asthma, COPD and Diabetic clinics / reviews and hold relevant certificates;
Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations (Adult & Child), assisting with minor surgery, dressing and wound care and recording spirometry;
Compassionate, empathetic approach and demonstrable kindness in all aspects of your work as well as valuing the dignity of our patients, relatives, carers and colleagues.
Knowledge and application of supporting long-term conditions.
Insight and understanding of current issues in nursing and NHS.
