This is an exciting opportunity for a Band 6 or experienced band 5 nurse or allied health professional to be a part of the falls team at LUHFT. The successful candidate will be responsible for clinical review and support of patients at risk of falling in hospital. They will be highly visible in the clinical setting promoting falls prevention.
The candidate will utilise advanced clinical skills and evidence based knowledge to assess patients with highly complex presentations and ensure individualized care management plans are implemented.
The successful candidate will contribute towards the continual development of professional evidence based practise in Falls Prevention, and act as a role model for all staff in the area of patient safety. Providing continuing education to trust staff.
The candidate will be responsible for the maintenance of own accurate written records in line with legal, professional and departmental requirements.
The candidate will use the Trust's IT resources to maintain accurate statistical information to be provided to Wards/Departments, Divisions, Trust and Commissioners.
To perform Specialist Falls assessment of patients who have sustained a fall or at a high risk of falling. Planning, implementing and evaluating care delivery using advanced clinical knowledge and clinical skills.
To act as the clinical expert and professional resource to the multi- professional team, patients and carers.
To contribute towards a multi-professional approach in the management of patients around falls prevention, especially in relation to ongoing care needs and discharge arrangements.
To establish and maintain effective communication with patients and carers /relatives.
To support patients and their families and ensure that they receive the required information to enable them to self manage, lead or participate in their care delivery.
To recognise changes in patients condition that require the intervention of others and refer on as appropriate.
To provide continuing education to trust staff in falls prevention in a range of differing teaching modalities.
To be involved in the review of patient falls incidents and the sharing of information in falls steering groups and governance meetings.
To participate in Trustwide and speciality audits and quality improvement projects to provide continued improvement of falls prevention.
To participate in the coordination of research studies and national audit projects.
