About the role:




As part of an exciting new service, we are looking to recruit 4 full-time Family Outreach Practitioners with a high level of passion and drive to work within a multi-disciplinary intensive support team that has been developed to support children and young people with mental health difficulties and/or intellectual Disabilities and Autism. Within the role you will offer support that will help these children and young people to improve their daily functioning and help us achieve our aim of delivering improved outcomes for children, young people and families in Bradford. The role will enable you to provide an enhanced function that supports the work of our social work teams in the Children and Families Trust and the work of our core CAMHS Teams. You'll be part of a Multi-Disciplinary team which includes a Consultant Clinical Psychologist, Nurse Specialist, Speech and Language Therapist, Occupational Therapist, Assistant Psychologist and Assistant Practitioner. Key Responsibilities:



Work directly with children and families in their own homes and communities, identifying the appropriate interventions/tools to deliver positive outcomes. This includes working early mornings, evenings and some weekends when the need arises. Contribute to holistic assessments of need and devise and implement appropriate plans for babies, children, young people, and families. Key work families presenting with a range of needs such as behaviours of distress, adverse childhood experiences, child and parental mental health, and complexities associated with the child's disabilities, to support and motivate parent/carers to make changes to improve outcomes for children and young people. Review our support package on a regular basis to track and show progress, including children, young people, families, and partner agencies as appropriate, working in partnership to achieve the best possible outcomes for children, young people and families. Maintain accurate up to date and appropriate records using electronic systems and equipment in line with GDPR regulations. Write reports and attend meetings, reviews court etc. as required, representing the department. Contribute to timely advice giving, screening and referral processes so families are supported at the right time and at the right level to make and sustain changes to keep their children and young people safe. Including with appropriate supervision making decisions and recommendations regarding the needs of children, young people, and families. Ensure the appropriate participation of children and young people in their own plans.

About you:



A minimum of 2 years continuous and recent experience working directly with children, young people and parents as part of a multi-disciplinary team to plan and support change in families. A relevant Level 3 professional qualification related to working with children, young people and parents and evidence of on-going training and continuous professional development (CPD) You will be able to manage your own workload and be committed to working with children, young people and their families across a range of settings, including within their own homes, to support improved outcomes Strong interpersonal skills. Experienced and expert in managing the delivery of quality outcomes for children and young people. An ability to build strong relationships with children, their families, colleagues and partner agencies. Embrace change and strive for excellent practice placing children and young people at the heart of decision making; you have experience of tackling the most complex needs with enthusiasm, drive, and determination. An experience and understanding of relevant statutory and legal requirements to safeguard children and young people
