Job Description

Martin House provides free specialist palliative care to babies, children and young people with complex and life-shortening conditions. Families from across West, North and East Yorkshire have access to our care 24/7, 365 days a year, including specialist doctors. Every year we care for more than 550 families, at the hospice, in hospital and in families' own homes, as well as supporting around 200 bereaved family members.


We are seeking a highly organised and skilled administrator for the role of Family Support Pathway Co-ordinator. This individual will work closely alongside our Family Support team, to ensure families have an excellent experience throughout their journey and maximise the team's impact.


The role will involve:


Providing organisation and oversight to the Family Support caseloads and services, thus maximising the team's impact and effectiveness. Provide a broad range of administrative support to the Family Support team. Assist the Family Support team in co-ordinating the programme of meetings, events, groups and activities. Maintaining an oversight and facilitating the patient journey from referral to discharge. Supporting the Head of Family Support with the organisation of scheduled audits, data collation and reports required. Supporting the Head of Family Support with information requests from the Income Generation team.
Our ideal candidate will


Be committed to Martin House's values - Excellence, Respect, Integrity and Compassion. Be a highly skilled administrator with a good understanding of Family Support services within palliative care. Have demonstrable experience of working in an administrative role in a busy environment where you are required to multi-task. Be able to use a full suite of Microsoft Office software systems Have experience of using a patient records system, such as SystmOne. Have experience of working in a role which involves coordination of services and/or caseloads. Be an excellent communicator who is able to interact with various internal and external stakeholders.
