Duties



Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately in compliance with HIPAA regulations Provide medication administration following prescribed care plans and ensure proper storage and documentation of all medications Prepare nutritious meals tailored to individual dietary needs and assist residents during meal times to promote independence and social interaction Maintain a clean, safe, and welcoming environment by performing regular cleaning duties in resident rooms and common areas Support residents with memory care needs, including dementia or Alzheimer's disease, utilizing specialized techniques to ensure safety and comfort Engage in patient observation to detect signs of distress or deterioration, promptly reporting concerns to healthcare professionals or supervisors Assist with transfers and mobility using appropriate equipment such as Hoyer lifts or resident's personal assistive devices Participate in care planning meetings and contribute insights based on daily interactions to optimize individual care strategies Follow all protocols related to hospice care, home health services, or working with individuals with developmental disabilities or autism

Qualifications



Proven experience in group home settings, assisted living facilities, nursing homes, or home & community care environments Knowledge of patient monitoring techniques, vital signs measurement, and medication administration procedures Certified in First Aid and CPR; additional certifications such as HIPAA compliance are preferred Familiarity with dementia care, Alzheimer's care, memory care techniques, and working with people with disabilities or developmental challenges Ability to perform heavy lifting safely when assisting residents with transfers or mobility needs Strong communication skills for effective social work interactions and teamwork within multidisciplinary teams Experience using care plans to deliver personalized resident support while adhering to safety standards and regulations
