We are seeking compassionate and dedicated Support Workers to provide essential assistance, guidance and support to a person in their own home with complex mental health conditions, in Durham.

The successful candidate will play a vital role in supporting the clients' independence, wellbeing, and daily living activities. This position offers an opportunity to make a meaningful difference within the community, working closely with the client to promote their choices and dignity.

There are days and night positions available.

Duties



Offer personalised support tailored to the individual's needs, including assistance with daily routines, life skills, de-escalation and safety. Develop and implement care plans in collaboration with healthcare professionals and the client. Provide emotional support and encouragement to foster confidence and independence. Maintain accurate records of support provided, progress notes, and any changes in client circumstances. Assist client with communication needs, ensuring they are heard and understood effectively. Support client in engaging with social activities and community resources to promote social inclusion. Work collaboratively with team members to ensure consistent and high-quality care delivery.

Skills



Excellent communication skills, capable of building trusting relationships with clients from diverse backgrounds. Basic IT skills for maintaining accurate documentation and updating care plans efficiently. Leadership qualities to motivate and guide clients towards achieving their personal goals. Ability to understand, interpret, and follow detailed care plans accurately. Empathy, patience, and a genuine desire to improve the lives of others. Organisational skills to manage multiple responsibilities effectively while prioritising client needs.

Requirements



Previous experience with Learning Disability, Autism and Mental Health conditions preferable. Strong communication skills in English, both verbal and written. Basic IT skills for record keeping and communication purposes.

A valid driving licence is essential for roles involving transportation of clients.

Ability to follow care plans accurately and adapt to the needs of individual clients. A caring attitude with a commitment to providing high-quality support to those in need.
This role is ideal for motivated individuals passionate about caring for others. Applicants should demonstrate patience, empathy, and a genuine desire to improve the quality of life for those they support.

Pay: 12.80-13.50 per hour

Expected hours: 16 - 36 per week
