We are seeking experienced, compassionate and dedicated Senior Support Workers to provide essential assistance, guidance and support to people in their own home with complex mental health conditions, in Durham, Murton, and Newton Aycliffe.
The successful candidates will play a vital role in supporting the clients' independence, wellbeing, and daily living activities. This position offers an opportunity to make a meaningful difference within the community, working closely with the client to promote their choices and dignity.
There are day and night positions available. Day positions require a full driving licence with use of own car to support people to access the community and attend appointments.
We do not currently offer sponsorship.
Duties
Offer personalised support tailored to the individual's needs, including assistance with daily routines, life skills, de-escalation and safety.
Develop and implement care plans in collaboration with healthcare professionals and the client.
Provide emotional support and encouragement to foster confidence and independence.
Maintain accurate records of support provided, progress notes, and any changes in client circumstances.
Assist client with communication needs, ensuring they are heard and understood effectively.
Support client in engaging with social activities and community resources to promote social inclusion.
Work collaboratively with team members to ensure consistent and high-quality care delivery by offering mentorship and guidance.
Skills
Excellent communication skills, capable of building trusting relationships with clients from diverse backgrounds.
IT skills for maintaining accurate documentation and updating care plans efficiently.
Leadership qualities to motivate and guide clients towards achieving their personal goals.
Ability to understand, interpret, and follow detailed care plans accurately.
Empathy, patience, and a genuine desire to improve the lives of others.
Organisational skills to manage multiple responsibilities effectively while prioritising client needs.
Experience working with children or adults with mental health conditions.
Ability to administer medication safely and responsibly
Excellent communication skills, both verbal and written
Compassionate approach with a strong commitment to client well-being
Organised, patient, and adaptable in dynamic environments
Ability to work effectively as part of a team or independently
