Job Summary
:
As a Support Worker, you'll provide specialist, compassionate, and high-quality care and support to individuals with primary diagnoses of mental illness, sex offenders, difficulties with substance abuse, and may also have a forensic history.
Your role includes offering direct support to clients, ensuring they comply with their treatment programs, attending professional appointments, supporting medication adherence, and assisting with all aspects of daily living (ADL). You'll work under the supervision of senior colleagues to formulate and implement clients' support plans and risk management strategies.
Key Responsibilities
:
Daily Living Skills: Assist clients with budgeting, cooking, and other daily living skills.
Personal Care: Provide physical support, including personal care when necessary.
Emotional Support: Address the emotional needs of clients.
Equitable Support: Work equitably with all clients, regardless of their needs or disabilities.
Community Engagement: Facilitate clients' involvement in social, educational, and employment opportunities in line with their care plans.
Home Management: Enable clients to participate in the daily running of the home, fostering responsibility for their well-being.
Rights and Benefits: Support clients in exercising their rights as UK citizens, including access to welfare benefits.
Household Duties: Perform household duties, ensure nutritious meals, and maintain a clean, orderly home environment.
Budget Management: Collaborate on managing the household budget.
Key Working: Develop and review individual support programs, attend Life Planning meetings, and ensure clients' active participation.
Communication: Maintain communication with family, friends, Care Managers, and other professionals.
Record Keeping: Keep administrative and recording systems up to date.
Holidays: Accompany clients on holidays and assist with communication.
Policy Adherence: Adhere to organizational policies, including anti-discrimination practices.
Health and Safety: Ensure compliance with the Health and Safety at Work Act.
Confidentiality: Respect the confidentiality of clients and the organization.
Team Collaboration: Foster a team-oriented environment.
Tenancy Support: Assist clients in understanding and maintaining their tenancy agreements.
Aids and Adaptations: Advise and support clients on aids and adaptations.
Housing Transitions: Support clients in transitioning to more suitable housing options.
Maintenance Reporting: Assist clients in reporting and recording maintenance issues.
Professional Conduct: Collaborate respectfully and courteously with colleagues.
CQC Compliance: Adhere to the Care Quality Commission framework.
Sleep-In Duties: Perform sleep-in duties as required.
Additional Responsibilities: Undertake additional responsibilities as defined by your Line Manager.
Basic education & Qualification:
NVQ Level 2 in Health and Social Care (desirable)
Experience:
Providing care
Supporting vulnerable individuals within the community
Supporting individuals with mental health issues
Working in a residential/supported living setting
Experience in Mental illness, Drug & Alcohol Addiction, and Offending History
