Horizon Healthcare is seeking an experienced First Contact Physiotherapist to provide high-quality musculoskeletal (MSK) assessment and management at the front door of primary care. You'll deliver same-day and routine appointments, empower self-care, and collaborate closely with our multidisciplinary team to improve patient outcomes and experience.
What you'll do
Provide first-contact MSK assessment, diagnosis and personalised management plans (including exercise prescription and self-management advice).
Apply red-flag screening, safety-netting and safeguarding; escalate appropriately to GPs or specialist services.
Request imaging and refer in line with local pathways and IR(ME)R requirements; signpost to community resources and social prescribing.
Maintain high-quality digital records in SystmOne, ensuring accurate coding and contribution to audits/QI.
Work closely with GPs, pharmacists, mental health practitioners, social prescribers and community services through regular MDT case discussions.
Support demand-management by ensuring patients access the right care at the right time.
About you - Essential
HCPC-registered Physiotherapist with substantial post-registration MSK experience (typically 5+ years).
Advanced clinical reasoning with confidence to practice autonomously within agreed pathways.
Clear, compassionate communicator able to explain diagnoses and management plans plainly.
Competent with SystmOne and digital tools; timely, accurate record-keeping.
Sound knowledge of safeguarding, consent/capacity, and EDI principles; ongoing CPD with familiarity to NICE and CSP guidance.
About you - Desirable
MSc/PGCert in Advanced Clinical Practice (Level 7) or working towards.
IR(ME)R training; prior FCP/ARRS experience in a PCN; contribution to QI and service development.
Contract specifics
Term: January 2026 start; end date: 31 March 2026.
Pattern: Monday, Wednesday, Friday, 09:00-17:00 (on-site at Burton Road).
Salary: Dependent on experience
Checks: Right to work, Enhanced DBS, OH clearance, professional indemnity, and two clinical references.
