Gwent Drug and Alcohol service provides support to individuals, families, carers, and others affected by substance use across all 5 local authorities. As part of our service, we are recruiting into a new specialist role Alcohol Support Workerthat aligns with the vision of the Gwent APB Substance Misuse Commissioning Team. We are seeking an empathic and dedicated individual with experience from a range of sectors to join us in establishing a new, innovative role within our service. We welcome people with experience of working not just in substance use, but in housing, mental health, primary care, secondary care, criminal justice, and other sectors that have transferableskills. We welcome individuals with their own lived experience of drug and alcohol use and/or mental health and from those who have supported others with lived experience.

Role Profile & Responsibilities
Role Purpose
To assist substance users who may be experiencing memory issues and complex needs to engage more successfully in health and social care systems.
To assess alcohol users who may be experiencing memory issues associated with Alcohol Use or Alcohol Related Brain Damage (ARBD)
To be competent in utilising the Mini Ace Assessment Tool (Training Provided)
Work closely with Hospital Liaison Team
Work closely with the Over 50's Alcohol Wellbeing Navigator
To provide support and seamless transition to specialist treatment services
To provide specific support to those with memory loss that do not meet the threshold for specialist services.
Main Duties & Responsibilities
Assist service navigation and flexible outreach provisions for service users with complex need and in association with memory issues.
Work closely with GDAS and ABDAS clinicalteams in providing a 'one stop shop' for memory loss issues and social care needs.
Person focused treatment plans.
These will include:
1:1 tailor made interventions
Groupwork that provides practical support in managing memory loss.
Produce care plans that are person centred and inclusive of supportive professionals, families, friends, and others.
Intensive case management
Helping to develop and maintain a social network. o Support to maintain a stable and independent home.
Conduct Treatment Outcome Profiles (TOPS)
Link with community services including GDAS Wellbeing College,to co-ordinate a holistic aftercare package.
Comprehensive and holistic exit planning
Signposting and onwards referral
Improve quality of care and health outcomes
Support with benefit claims where conditions are degenerative and access support.
Link with adult social care teams and other relevant professionals.
Ensure transitional support is provided when attending healthcare appointments.
Work closely with ABSDAS specialist workers to ensure appropriate support
Harm Reduction Provision
Deliver harm reduction services and advice, including provision of Prenoxad Training, Needle Exchange Program (NEX) and Dry Blood Spot Testing (DBST).
Compliance and Information Management
Ensure compliance with internal case management systems and client records.
Undertake appropriate monitoring, review, and evaluation procedures.
Adhere to host organization and GDAS policies and procedures including safeguarding, GDPR and professional boundaries.
Undertake appropriate administrative tasks.
Networking and Awareness Raising
Foster and sustain good working relationships with stakeholders, including local partners within the local authorities and the health board.
Represent host organization or GDAS in appropriate forums and groups.
Deliver talks, workshops, presentations and provide training where appropriate.
Development & Training
Provide a lead role in an area related to Memory Loss and Alcohol Related Brain Damage (ARBD) (full training will be provide)
Engage in a range oftools and methods designed to support further development in practice, including, but not limited to Memory Loss and Alcohol Related Brain Damage.
Participate in Supervision, Reflective Practice, and Group Supervisions.
Commitment to Learning and Practice Plans, ensuring knowledge and training. remain up to date with all safeguarding practice and legislation. Take up arrangements for support and participate in all relevant internal and external training Other
Ensure sound joint working practice to deliver an integrated seamless care pathway by working as required with other GDAS disciplines and external partners.
Communicate host organization and GDAS vision and deliver upon mission, corporate strategy and business plans.
