Job Description

Duties and Responsibilities are:

The post holder will make decisions, provide safe, evidence-based, cost effective, individualised patient care. Clinical : To undertake all duties inline with the training and instruction provided by the general practice team. Responsible for the management of patients with LTC(s) by ensuring the smooth running of chronic disease clinics, including clinical, emotional, physiological reviews and support Responsible for clinical and administrative pathways to provide patients with continuity, coordinated, informed, planned care Assist patients with their medications, compliance and check any equipment used, such as inhalers, are working effectively Responsible for QoF and IIF targets and maintain a high level of achievement Undertake routine tasks related to patient care, including but no exhaustive: Venepuncture Blood pressure monitoring 12 lead ECG's Carry out baseline observations such as pulse oximetry, temperature, pulse rate Height Weight BMI Removal of sutures/clips Basic wound care/dressings The testing and result recording of clinical specimens, including urinalysis New patient health checks Vitamin B12 injections Influenza and Pneumonia vaccinations Chaperone Ensure specimens are recorded and ready for onward transportation including blood centrifuge The ongoing cleaning of clinical equipment and working surfaces Ensure adequate infection control procedures in consulting rooms Participate in any relevant health promotion and screening activities Maintain accurate and contemporaneous records Administration: Assist with the call/recall system for LTC management Arranging appointments, referrals, tests and follow up appointments as required Work closely with the care co-ordinators and social prescribers Maintain the store and treatment room this includes ensuring the room is always clean and tidy Assist with monitoring and recording of fridge temperatures Ensuring fridges are cleaned routinely in accordance with guidelines Attend and participate in any practice meetings when required Communication : The post holder should recognise the importance of effective communication and will strive to Communicate effectively with patients and carers to ensure they are fully informed Utilise effective communication skills to support patients Provide support, guidance and information in a format that meets the needs of the patient Communicate effectively with other team members and external stakeholders Act as an advocate when representing the patients and collegues viewpoints to others Fully participate with team meetings, team activities and delivering the nursing agenda Equality and Diversity : The post holder will support the equality, diversity and rights of patients, carers and colleagues, to include: Acting in a way that recognises the importance of peoples right, interpreting them in a way that is consistent with practice procedures and policies and current legislation Respecting the privacy, dignity, needs and beliefs of patients, carers and colleagues Behaving in a manner which is welcoming, non-judgemental and respects circumstances, feelings and rights Identify patterns of discrimination, take action to overcome this and promote equality Personal and Team Development : The post holder will participate in any mandatory and statutory training programme implemented by the practice as part of this employment. Actively participating in ongoing learning and development to include An annual individual appraisal, including taking responsibility for maintaining a record of own personal and professional development Taking responsibility for own development, learning and performance Understand own responsibilities and accountability in the delivery of GPA services to patients Disseminate learning and information to other team members in order to share good practice Provide educational role to patients, carers, families to facilitate learning Team Working : The post holder will work as an effective and responsible team member, supporting others and developing new ways of working.



To include Work effectively with others to clearly define values, direction and policies impacting upon care delivery Ensure clear understanding and utilisation of referral mechanisms within the practice Accept delegation from other nurses, prioritise own workload and ensure effective team management Participate in and support local projects as agreed within the practice team Management of Risk : The post holder will work to effectively manage risk including Monitor work areas and practices to ensure they are safe, free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines Ensure safe storage, rotation and disposal of medication, sock and equipment Apply infection control measures within the practice according to local and national guidelines Confidentiality: The post holder will work effectively to ensure confidentiality to all. To include consideration of In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately In the performance of the duties out lined in this job description, the post holder may have access to confidential information relating to patients and their careers, practice staff and other healthcare workers.
