The Elms Medical Practice is well-established, purpose-built GP surgery located to in Chester serving approximately 10000 patients.

Main duties of the job
Management of chronic disease -monitoring and development of individual care plans, agreeing these as appropriate with the patient and other health professionals.
Confident in Respiratory and Diabetes management.
Family planning & woman's health including cervical smears.
Travel health advice and vaccinations.
Child immunisations and advice.
QOF, DES & LES Implementation.
Qualifications
- Experience of proactive working in a GP surgery setting
- Proficiency in developing and implementing care plans for patients
- Strong patient care skills with a focus on empathy and compassion
- Qualified RGN/RN/ NMC Registered
Applicants must have current UK professional registration.
Job Type: Fixed term contract
Contract length: 5 months
Pay: From 22.00 per hour
Expected hours: 15 - 20 per week
