Job Description


Greenwell PCN is looking for a dynamic individual to join their multidisciplinary team as a salaried GP for up to 8 sessions per week working across 7 practices.
We are a small sized PCN in Greenford & Hanwell, a suburban area, with a strong emphasis on patient care through evolution, improvement and relationship building. We have a PCN of 7 GP practices and are supported by two clinical directors, two PCN managers and a care coordinator. Our PCN team consists of social prescribers, clinical pharmacists and physiotherapists. We have also developed additional services through our PCN which include a HIU hub, child health hub, womens clinic and family link team.
As a General Practitioner (GP) employed through the Additional Roles Reimbursement Scheme (ARRS) within a Primary Care Network (PCN), you will provide high-quality, patient-centred care while working as part of a multidisciplinary team. This role is ideal for a recently qualified GP, offering the opportunity to develop your clinical skills and confidence in a supportive environment.
You will manage a range of acute and chronic conditions, participate in health promotion and contribute to team-based care plans, with appropriate supervision.
The post encourages professional growth through regular feedback, mentoring and opportunities for further training in line with early-career development.
Main duties of the job
The GP will undertake a variety of duties governed by the GMS contract and objectives of Enhanced services. including
In person consultations
Telephone consultations
Checking and signing repeat prescriptions
Dealing with queries, paperwork and correspondence as required
Clinical administration
Working closely with our PCN and practice teams
Wider Responsibilities
Make professional, autonomous decisions in relation to presenting problems.
Screen patients for disease risk factors and early signs of illness
Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate
Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICE
Ensure appropriate follow up of patients
Record clear and contemporaneous computerised consultation notes to agreed standards
Participate in clinical audits, and learning event analyses and practice meetings
Compile and issue computer-generated acute and repeat prescriptions avoiding hand-written prescriptions.
Prescribe generically whenever appropriately and in accordance with the Practice prescribing formulary, NICE guidance and Good Prescribing Practice.
Work with and provide support for other members of the primary health care team
Play a lead role in the practice achieving agreed performance indicator targets

Undertake additional training and development to close potential gaps in skills and knowledge

Ensure the confidentiality and security of all information that is dealt with in the course of performing duties
Ensure that all systems for Information Governance, GDPR, and confidentiality is maintained at all times

  • Qualified GP with full GMC registration licence to practice, on a performers list Newly Qualified General Practitioner (less than 2 years from qualification)
