We are seeking a motivated and clinically skilled pharmacist to join our GP practice team. You will play a key role in delivering high-quality, patient-centred care by supporting medicines optimisation, conducting structured medication reviews, and contributing to the management of long-term conditions.
Key Responsibilities:
Conduct structured medication reviews (SMRs) for patients with polypharmacy, frailty, or long-term conditions.
Support repeat prescription management and ensure safe, evidence-based prescribing.
Provide clinical advice to patients and staff on medicines use and minor ailments.
Reconcile medications following hospital discharge or outpatient appointments.
Contribute to QOF, PCN DES, and other quality improvement initiatives.
Liaise with community pharmacies and secondary care to ensure continuity of care.
Participate in audits, formulary reviews, and implementation of NICE guidance.
Support the practice in achieving prescribing targets and cost-effective use of medicines.
Essential Criteria:
MPharm degree or equivalent
Strong clinical knowledge and communication skills
Ability to work independently and as part of a multidisciplinary team
Desirable Criteria:
Independent Prescriber qualification (or working towards it)
GPhC registration as a pharmacist
Experience in primary care or general practice
Familiarity with EMIS
Understanding of NHS prescribing frameworks and local formularies
