Our focus is on delivering innovative, high-quality services that address population health needs -- from urgent care and diagnostics to long-term condition management. The Respiratory Diagnostic Hub is central to our mission of improving early diagnosis, enhancing patient self-management, and reducing avoidable hospital admissions.

Role Overview

We are recruiting a GP with Extended Role (GPwER) in Respiratory Medicine or a Specialist Respiratory Nurse to lead clinical review and management planning for patients attending our diagnostic hub.

You will be responsible for confirming or ruling out asthma diagnoses, providing expert clinical assessment, developing personalised asthma management plans, and supporting patient education.

This role is ideal for clinicians passionate about respiratory care who want to work flexibly across community and primary care settings, contributing to high-impact population health improvement.

Develop and communicate comprehensive asthma management plans, ensuring personalised, evidence-based recommendations. Prescribe or recommend appropriate inhaler therapy, stepping up or down as clinically indicated. Deliver patient education sessions focusing on inhaler technique, trigger avoidance, adherence, and self-management. Collaborate with PCN pharmacists, practice nurses, and SmartCare admin teams to ensure effective follow-up and continuity of care. Provide clinical supervision or mentorship to diagnostic clinicians and support staff where appropriate. Contribute to service evaluation, audit, and quality improvement initiatives within the Respiratory Hub. Maintain compliance with SmartCare's clinical governance and data protection policies.

Proven ability to work collaboratively within a multidisciplinary environment.
Desirable Skills & Attributes

Experience in community diagnostic or respiratory assessment clinics. Competence in interpreting complex spirometry traces and FeNO results. Interest in clinical leadership, pathway development, or service redesign. Commitment to reducing health inequalities and improving access to high-quality respiratory care.
