Background
Social Prescribing empowers people to take control of their health and wellbeing through a referral to a Youth Wellbeing Mentor. Youth Wellbeing Mentors are able to spend time with an individual, focusing on 'what matters to me' and taking a holistic approach. They connect people to community-based and community-led social, practical and emotional support, as well as statutory services.
Social Prescribing aims to increase people's active involvement in their local communities. It can strengthen resilience at both a personal and community level, and it reduces health inequalities by supporting people to address the wider determinants of health, such as debt, poor housing and physical inactivity. It works particularly well for adults, children and young people who are lonely or isolated, and those with complex social needs which affect their wellbeing.
A key element of the support offered by the Youth Wellbeing Mentor is to build clients' motivation and confidence for change. They do this by utilising person-centred and asset-based approaches such as motivational interviewing, coaching and resilience building. They work to increase social connectedness, increase physical activity, improve financial stability, improve personal safety and support the development of social networks and community assets. Our Youth Wellbeing Mentors enable clients to achieve their goals through one-to-one support.
Nationally, and locally here in Derbyshire, social prescribing for adults is developing fast and delivering fantastic results. Traditionally it has been used to help adults and older adults find non-medical ways of combating loneliness and stress, but social prescribing has huge potential for young people too.
In 2021, Chesterfield Football Club Community Trust and Derbyshire Voluntary Action worked in partnership to establish a social prescribing service for children and young people in Chesterfield. Thanks to increased support from funding partners we are able expand our team of Youth Wellbeing Mentors to provide even more community-based support for 14-18 year-olds (up to 25yrs with additional needs).
Job Purpose
To work within the Thr1ve organisation, supporting Young People across North Derbyshire through group delivery and support. The main focus of this role is to support on the delivery of existing groups that are offered across the area both in schools and out in the community. Delivering groups on a range of different topics, from Young People Mental Health sessions or Skills Development sessions within schools, and community based Sport & Exercise sessions & Social Groups. As part of this role you will also be required to record and monitor data around the sessions and provide accurate evaluation reports where necessary. Along with some scope for the adaptation of existing group or the development and implementation of new groups where the service has identified gaps within the community.
Key Responsibilities
Group Development & Youth Engagement
To support staff on the delivery of a wide range of groups.
Where appropriate, co-design new youth groups in collaboration with mentors and young people, ensuring inclusiveness and accessibility.
Build and maintain rapport with Young People, Parents, Carers.
Partnership Working
Identify and engage local organisations aligned with the group's purpose.
Maintain strong relationships with existing partners and develop new relationships with identified key service providers.
Training & Capacity Building
Deliver basic training and mentoring to young people on leadership, participation, and group skills.
Support adult volunteers or staff in developing skills to sustain and lead youth groups.
Monitoring & Learning
Track progress and impact using case studies, feedback, and monitoring frameworks.
Contribute to reporting for funders and stakeholders.
Share learning to inform future youth engagement and group development work.
Person Specification
Essential
Experience working in a supportive role with children and young people (including in a peer support capacity)
Level 3 qualification (achieved or working towards) in relevant subject or extensive experience
Sound understanding of the role, benefits, and impact that pro-social community activity can have in improving health and wellbeing
IT skills - ability to accurately record data and information.
A 'people person' able to communicate effectively and build rapport with young people & young adults
Ability to provide support in a way that engenders trust, confidence, and enthusiasm
Ability to maintain effective working relationships and to promote collaborative practice with colleagues and stakeholders across multiple organisations
A creative and innovative mind-set, and assets-based approach to developing the potential of young people & young adults
Knowledge of and ability to work to policies and procedures, including confidentiality, safeguarding, information governance, and health and safety
Willingness to work flexibly, including some evening and weekends, to meet the demands of the project
Full Driving Licence
Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions

Sound understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on young people and communities
