About the role
The Head of Care supports the organisation by overseeing the delivery of daily care and support for the residents by supervising the care and liaising with the Registered Manager. Managing the staff to ensure the delivery of care and support meets both CQC and local authority quality standards. Being responsible for updating care plans to reflect the residents needs and ensure their health and well being is maintained. This will include working with health and allied professionals outside of the organisation to provide the best personal centred care delivery.
Responsibilities
Manage the daily delivery of personal care for the residents to maintain all resident's health and wellbeing in alignment with their personal care plan, by supervising the care and support team. Allocating staff's workload on a day to day basis to ensure the residents needs are meet
Manage the medication administration to residents when on shift ensuring that all staff delivering medication are competent. All medication is administered in alignment with the company's policy and the individuals care plan and prescription.
Liaising with health and allied professionals associated with individuals attending MDT's and arranging appropriate appointments to maintain the health and well being of each individual resident. Ensuring review and appointment dates are managed and residents are supported with their appointments.
Ensuring everyone is supported to maintain family contact, updating family/friends of changes in the individuals care.
Overseeing the rota when on shift to ensure that staffing levels meets the organisations expectation reporting concerns to the Registered Manager and seeking authorisation to use agency.
Reporting any concerns about individual's safety to safeguarding, and the Registered Manager/Nominated Individual in alignment with legal standards.
Driving forward mandatory training to ensure that each member of staff continues to achieve their CPD
To work with the HR Manager to ensure new staff members are inducted and shadowed with the appropriate member of staff. Liaising with the Learning & Development Consultant with any identified training needs or request from the observation and supervision mechanism.
To carry out supervisions and oversee staff's performance informing the Senior Management team of concerns in performance and the methods used to work with the staff concerned.
Ensure that care plans and record keeping is kept up to date evidencing the delivery of care, reporting of incidents and completing documentation in alignment with company policy and procedures.
To admit new residents ensuring that their care needs are managed, including following the admission plan - beds are ready - medication is booked in, height and weight carried out.
Liaising with kitchen team reporting changes in the individual's dietary needs or any concerns about an individual's nutritional needs.
Carrying out room checks and health and safety checks on individual rooms and throughout the home to make sure there are no areas of risk for both individual and staff. Reporting concerns to the maintenance team.
Liaise with the cleaning team to ensure that required standards are met, and issues are dealt with promptly.
Work with the wider team and Kitchen regarding actives both in house and those in the community that residents wish to attend.
To attend a Head of Department meeting on a weekly basis to raise concerns and put in place any changes to ensure the residents needs are met.
To work as part of the on-call rota ensuring the home is safe and staff are supported to manage the home.
To undertake observations and checks on the work of the care and support staff to ensure they are working in alignment with company policies and procedures and CQC requirements.
To attend and undertake training as the company requires.
