Head of Programme - Integrated Community Care System (ICCS) Programme
. Reporting to the Deputy Director for Futures & Integration
The Social Services and Integration Directorate is responsible for the development and implementation of policy and strategy for social care and integration in Wales.
The post is situated in the Partnership and Integration Branch within the Social Services and Integration Directorate.
The main functions of the branch are aligned to the Social Services and Wellbeing (SSWB) Act 2014 and underpinned by the following principles:

The main responsibilities of the Futures and Integration Division include:
Delivery and evaluation of the 146m Regional Integrated Fund (RIF) through Regional Partnerships Boards (RPBs).
Oversight of the SSWB Act 2014 Part 9 - Partnership Development.
Children and Young People's integrated models of care. With oversight of NEST/NYTH framework within the Regional Partnership Boards.
Oversight of the evaluation and integration of the new national 6 integrated Modules of Care:
Community based care: prevention and community co-ordination.
Community based care: complex care closer to home.
Promoting good emotional health and well-being services
Supporting families to stay together safely, and therapeutic support for care experienced children
Home form Hospital services, and
Accommodation based solutions
Setting outcomes and measures for integration.
Undertaking international research and benchmarking.
Oversight of health and social care integrated hubs.
Delivery of the Integrated Community Care Service (ICCS) Programme, including digital and Connecting Care.
Oversight of Continuing NHS Healthcare (CHC) and Funded Nursing Care (FNC) policy.
Oversight of the Information, Advice and Assistance (IAA) services in Wales, delivered within Local Authorities.
Oversight of the Connected Communities - tackling loneliness and isolation strategy.
Oversight of the SSWB Act 2014 Part 2 - Co-production, Section 16 and rebalancing the market - including citizen voice.
Oversight of policy relating to Learning Disabilities and Neurodivergence.
Working across the broader division and the wider health, social care and housing directorates, you will be responsible for leading the programme of work to deliver an Integrated Community Care System for Wales, to deliver the ambitions set out in A Healthier Wales.
This role encompasses both leadership, policy and delivery work.
The ability to work collaboratively across different sectors in a complex policy landscape is key for this role.
The post holder will need to work flexibly and closely with other teams across government including health, social care, housing, education and local government, as well as with Ministers and external partners.
The main responsibilities for Integrated Community Care System (ICCS) Team
This team will support the Health and Social Services Group to realise the ambitions set out in A Healthier Wales to develop and implement an Integrated Community Care System (ICCS) for Wales.
They will provide leadership and programme management support for the programme as a whole but will also lead on some specific work packages to ensure delivery against the ICCS Blueprint. Their remit will include;
Providing programme leadership and co-ordination including monitoring and reporting.
Facilitating and providing secretariat to the programme delivery structure including the ICCS Leadership Group, the Senior Officers' Engine Room, wider stakeholder engagement groups, and the Care Action Committee.
Overseeing, monitoring and reporting on the investment of the 11.95m 'Further Faster' funds
Co-ordinating and supporting alignment between the range of connected national programmes to ensure delivery of the ICCS for Wales, including the RIF, UEC 6 Goals programme, the Strategic Primary Care Programme, and the Integration and Rebalancing Capital and Housing with Care Funds.
Leading on engagement and communications in relation to the ICCS both internally and externally, including direct support for the RIF/IRCF/HCF funds.
Provide co-ordination and policy leadership for;

Head of Programme - Integrated Community Care System (ICCS)
This is a new post within the Division. The post holder will be responsible for supporting the Deputy Director of Futures and Integration, along with their own specific responsibilities and deliverables which include:
Day-to-day management of the ICCS Programme.
Lead on the further development and implementation of the Integrated Community Care System for Wales, which includes the following components:
Support the policy development and implementation in relation to the Programme for Government commitment to establish 50 integrated health and social care hubs across Wales. This will entail working alongside colleagues in the capital/estates teams who manage the capital programme processes of the Integrated and Re-balancing Capital Fund.
Liaise and work with other members of the Futures and Integration Team who lead on other integration related work including the Regional Integration Fund.
Be a link within the Connecting Care work, a digital community care record to replace WCCIS.
To establish programme management structures and reporting systems.
Promote and support improvement in regional and national effectiveness to deliver the ICCS.
Line-management of the ICCS Team
Drafting and contributing to Ministerial advice and briefings in relation to ICCS and the development of integrated health and care services
