We are currently looking for dedicated and compassionate individuals to join our team as Door-to-Door Health Care Assistants.

Job Summary:


As a Door-to-Door Health Care Assistant, you will provide in-home care services to Clients in the community. You will assist individuals with personal care, daily living tasks, and medical support. Your role will require you to visit Clients' homes on a scheduled basis, ensuring their health and well-being while delivering top-tier customer service.

Key Responsibilities:



Personal Care Assistance:

Assist Clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support.

Health Monitoring:

Regularly monitor Clients' vital signs, including blood pressure, pulse, and temperature. Record and report any significant changes to the healthcare team.

Medication Assistance:

Help clients take their prescribed medications according to their care plan. Ensure proper documentation and timely reminders for medication administration.

Companionship:

Provide emotional support and companionship to clients. Foster a supportive and encouraging environment to enhance the quality of life for clients.

Transport Support:

Assist clients with transportation to medical appointments and other essential errands as needed.

Care Plan Adherence:

Follow individualized care plans and ensure that the services provided are in line with the clients' medical and personal needs.

Record Keeping:

Maintain accurate clients communication records, including daily activity logs, health monitoring notes, and any observations that need to be communicated to the medical team.

Communication:

Establish and maintain open communication with clients, family members, and healthcare professionals to ensure the best possible care.

Household Assistance:

Support clients with light housekeeping, meal preparation, laundry, and other activities that promote a safe and comfortable home environment.

Qualifications:



1-year Previous experience in health and social care, home care, or a related field is preferred.

Valid driver's license with reliable transportation to travel between clients' homes.

Strong understanding of personal care procedures and the needs of elderly or disabled individuals.

Ability to maintain confidentiality and adhere to HIPAA standards.

Excellent communication and interpersonal skills.

Compassionate, clients, and empathetic nature with a genuine desire to help others.

Ability to effectively manage a busy schedule, including traveling between patient homes.
