Lea Vale Medical Group are looking for a caring and motivated Healthcare Assistant to join our friendly primary care team. We have an excellent nursing team of Practice Nurses, Nursing Associates and Healthcare Assistants and you will be joining a supportive and welcoming practice.

Salary - Equivalent to Agenda for Change Band 3 or 4 depending on experience

Main duties of the job



You will be assisting trained nurses and other health care professionals in the delivery of the highest standard of patient care, ensuring that this is sensitive to the needs of the patient.

You will manage your own clinics, delivering long term condition checks, dressings, blood tests, etc. both in practice and where required in patients homes

Job responsibilities

Patient Care



Undertake routine diagnostic nursing tasks such as: ECGs, Blood Pressure checks, Blood Glucose monitoring, routine heights, weights and urinalysis Provide wound management for the patients under the guidance of registered nurses. Complete health assessments on newly registered patients and document simple medical history. Use patient contacts as opportunities to promote health education, including education on practice systems such as obtaining a blood result. Undertake NHS Health Checks, using the opportunity to promote health and well being Complete long term condition monitoring with appropriate training Act at all times with regard to the confidentiality and sensitivity of issues to do with patients, their care and treatment. Record and document all patient contact.

Governance



Adhere to the nursing protocols in place, and contribute to Practice audits Participate in programmes of continuing professional development and training under the guidance of the Lead Nurse Attend clinical and team meetings and tutorials as required Undertake mandatory and statutory training

Qualifications



Desirable

NVQ level 3 in Health and Social Care Managing long term conditions training Phlebotomy certificate

Experience



Essential

Experience of working with the general public Experience of working in teams under appropriate supervision as part of a multi-disciplinary team Insight into how to evaluate own strengths and development needs, seeking advice where appropriate Experience of providing and receiving complex, sensitive information Detailed knowledge and understanding of appropriate procedures and treatment plans
Desirable

HCA experience in primary care

Basic Skills & Abilities



Essential

Ability to work autonomously and as a team member Excellent communication skills Competent in venepuncture and recording clinical observations Ability to take part in reflective practice and clinical supervision activities Ability to deal with non- routine and unpredictable nature of workload and individual patient contact Ability to communicate effectively (written, verbal and non verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team. Ability to develop effective and appropriate relationships with people, their families, carers and colleagues Knowledge of and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance and health and safety
Desirable

Competent in wound care. Experience of SystmOne
