Health Care Assistant - Community Integrated Response Team/Home First

This is an exciting opportunity to be part of Wirral's Community Integrated Response Team, where you be supporting people to remain independent in their own homes, using a reablement approach to help them reach their full potential.



Your daily tasks will be varied and include supporting people with activities of daily living, personal care, moving and handling, prompting medication, meal preparation and social care and support, working with therapists and social workers to make sure people are achieving their recovery goals.



Care is provided to patients within their own homes.



To join our multidisciplinary team you will:



Have an NVQ Level 3 in Health and Social Care, or demonstrable equivalent experience



have a caring attitude



have a strong desire to help people



have good communication skills



The post involves a 7 day working rota .

About the Community Integrated Response Team (CIRT)



CIRT is an integrated team of clinical staff and professionals including; Nurses Physiotherapists, Occupational Therapists and Social Workers. The team have a strong focus on admission prevention and early supported discharged from ED and assessment areas. One of the fundamental principles of the Healthcare Assistant Role is to support our patients with their reablement and self care, encouraging them to reach their optimum level of independence.

This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable.



We are looking for candidates with experience of providing reablement, therapy and/or care in the community and working as part of a multi-disciplinary team.



The job role is required to work over a 7 day shift pattern, 8am - 10pm, Monday - Sunday.



To assist registered nurses / therapists when they are implementing assessed care needs and help to maintain progress in rehabilitation / reablement.



To assist patients / individuals with personal care tasks, which is delegated as part of the reablement care plan as well as any other additional patient care duties. This will include assistance with meals and drink preparation if required.



To provide and assist with the care of people in their homes, working with individuals and their families to ensure a person-centred approach including the participation in clinical care procedures e.g., recording observations and application of topical cream



To support admission prevention by responding to incidents of carer breakdown, provide rapid support for individuals to prevent admission to hospital.
