We are seeking a compassionate and dedicated Healthcare Assistant to join our team. The ideal candidate will play a vital role in providing high-quality care to patients in both home care and care home settings. This position requires effective communication skills, the ability to follow care plans, and a commitment to enhancing the well-being of those we serve.
Duties
Assist patients with daily living activities, including personal hygiene, dressing, and mobility.
Follow individual care plans to ensure that each patient's needs are met effectively.
Communicate clearly with patients, families, and healthcare professionals to provide updates on patient status and any changes in condition.
Maintain accurate records of patient care and report any concerns to the supervising nurse or healthcare professional.
Support patients in engaging with their community and participating in social activities.
Drive to various locations as required for home visits or patient appointments.
Utilise IT systems for documentation and communication purposes.
Skills
Strong ability to communicate effectively in English, both verbally and in writing.
Experience or knowledge in creating and following care plans is advantageous.
Previous experience in home care or care home environments is preferred.
A valid driving licence is essential for this role.
Proficiency in IT skills for managing patient records and communication is beneficial.
