Job Description

Jobpurpose To support delivery of safe, effective, person-centred care across routine clinics and long-term condition pathways under the supervision of registered clinicians. To contribute to prevention and health promotion initiatives, vaccination/immunisation programmes, and reduction of health inequalities across the practice population. To maintain high standards of infection prevention and control, accurate clinical documentation, and good patient experience. Key responsibilities Clinical (under supervision and within competence) Perform observations and basic clinical measurements: BP, pulse, SpO2, temperature, height/weight/BMI, peakflow.



Phlebotomy and specimen handling; urine dipstick and point-of-care tests as trained. ECG recording. NHS Health Checks and lifestyle brief interventions (smoking cessation,alcohol screening, weight management, physical activity advice). Wound care support: simple dressings, suture/clip removal, leg ulcer care under protocol.



Vaccinations and immunisations support within scope and training. Chaperoning and assisting with procedures(e.g.,smear clinics,minor surgery). Long-term condition recall support: gather QoF data, pre-clinic tests, footchecks (as trained), inhaler technique, BP optimisation support. Health promotion, signposting to PCN services (social prescribing, weight management, smoking cessation).



Clinical governance and safety Work to practice SOPs; escalate red flags promptly to a registered clinician. Maintain accurate records. Maintain stock, equipment calibration, cold chain support, and room readiness. Comply with infection prevention and control, waste disposal, sharps safety.



Participate in audits and quality improvement projects. Safeguarding: identify and escalate concerns in line with policy; record accurately. Administration and teamwork Prepare clinics, manage recalls,sendpatientcommunications(SMS/letters),andsupportdataquality/QOF. LiaisewithMDTcolleagues(GPs,PracticeNurses,ClinicalPharmacists,SocialPrescribers,Physiotherapists,MentalHealthpractitioners).



Supportequitableaccessforvulnerablegroups(housebound,carers,LD,homelessness,non-Englishspeakers)withreasonableadjustments. Scopeandsupervision Workunderthedirectionofaregisteredclinician;donotworkbeyond competence. Seek guidance when unsure; participate in supervision, debriefs, and annual appraisal. Maintain confidentiality and up hold GDPR/IG requirements.



Training, development and appraisal Statutory/mandatory training. Under take role-specific training (phlebotomy, ECG, woundcare, NHS Health Checks, vaccination support). Participate in CPD and competency sign off; track learning against the Primary Care and General Practice Nursing Career and Core Capabilities Framework. Skills:

BP/obs, phlebotomy, urine dipstick, ECG, accurate documentation. IT: Confident with clinical systems Communication: Clear, compassionate communication; able to use interpreters and accessible information when required. Teamworking: Works well within an MDT; understands professional boundaries; reliable and organised. Governance: Understandsconfidentiality,consent,infectioncontrol,andsafeloneworkingwithinpolicy.
