We are seeking a compassionate and dedicated Health Care Assistant to join our team. The ideal candidate will provide essential assistance to individuals in need of care, ensuring their comfort, safety, and well-being. This role is pivotal in enhancing the quality of life for those we support, whether in a residential setting, nursing home, or through home care services.
Responsibilities
Provide patient care and support to individuals in various settings, including senior care and assisted living facilities.
Assist with daily living activities such as bathing, dressing, and meal preparation.
Administer medication as prescribed and monitor health conditions.
Engage in behaviour management techniques to support individuals with challenging behaviours.
Perform heavy lifting when necessary to assist clients with mobility or transfers.
Maintain a clean and safe environment for clients by adhering to health and safety regulations.
Collaborate with healthcare professionals to ensure comprehensive care plans are followed.
Document care provided and report any changes in client condition to the appropriate personnel.
Service user Description
Service user a 23-year-old man residing with his mother, is a college student attending Avelon Road Day Opportunities Centre five days a week. He relies on taxi transportation and an escort for his commute due to health reasons. Following a recent kidney transplant from his mother, he now has a flexible college schedule that allows for in-person attendance once a week, with the option of online classes when needed for rest and recovery. Service user has impaired hearing and is awaiting hearing aids. While he communicates with familiar individuals, he may struggle with strangers and often depends on others to anticipate his needs. He experiences fatigue, requires assistance with weight bearing due to unsteadiness and poor spatial awareness, and manages pain and discomfort from hip dysplasia. Community outings necessitate a wheelchair for mobility support. Managing multiple complex health conditions, including Partial Trisomy 22, hypertension, developmental delay, kidney disease, and gastrointestinal issues, has a history of surgeries and ongoing medical interventions. He currently maintains a
PEG feeding tube, ileostomy, and medications,
with a regimen overseen by his health care assistant. Hygiene needs, skin care, and stoma maintenance are part of his daily care routine. Emotionally, service user occasionally experiences tear fulness, requiring support. Despite these challenges, he exhibits overall stability and manages his oral intake alongside PEG feeding and hydration needs.
